Job Description

Occupational Therapist Community Care Coordinator In accordance with the Vision, Purpose, and Values of the Vancouver Island Health Authority, patient safety is a priority. The Community Care Coordinator OT (CCCOT) provides care within a clientcentred approach, assessing, planning, delivering, organising, reviewing, and evaluating the care needs of clients and caregivers requiring communitybased services. The CCCOT fosters continuity of care, collaborates with physicians and other health professionals, provides education and counselling to clients and families, and works autonomously within an interdisciplinary model. Qualifications Education, Training and Experience Graduation from a recognised university program in Occupational Therapy; current licensure with the College of Health and Care Professionals of BC as an Occupational Therapist; eligible for membership in the CAOT. Three years of recent related experience with complex disabilities, including two years of community health experience with a homecare component, or equivalent. Valid BC Drivers Licence. Skills and Abilities Ability to integrate and apply COTBC Standards in community practice. Knowledge of community health, Expanded Chronic Care Model, health promotion, illness prevention, and complex disabilities. Understanding of lifespan development, family theory, multicultural health practices, health education, and determinants of health. Demonstrated ability to assess, analyse, plan, prioritise, evaluate, and communicate occupational performance. Strong written and verbal communication, facilitation, mediation, conflict resolution, critical thinking, and problemsolving skills. Ability to establish effective working relationships with clients, families, and interdisciplinary team members. Preparation and maintenance of charts, records and reports. Discharge planning and case coordination. Effective participation in purposeful change. Physical competence to perform job demands. Basic computer literacy, including email, internet and Continuing Care Information Management Systems. Knowledge of chronic disease management. Knowledge of other health disciplines and their role in client care. Effective communication, negotiation and collaboration with coworkers, physicians and other health staff.
