Pharmacist 2 - Renal Unit Pharmacy Location: Richmond Hill, Ontario, Canada Summary Of Duties To provide a full range of pharmaceutical care for patients living with Chronic Kidney Disease throughout the continuum of care (inpatients/outpatients) at an advanced level. Responsibilities Include, But Not Limited To Medical history and medication reconciliation Physician order review, clarification, and entry Participation in multidisciplinary dialysis rounds and kidney care clinics Answer drugrelated questions and provide patient counseling Supervise and support renal pharmacy technicians Lead the development, implementation, and maintenance of patient education materials, medication policies, procedures and protocols The successful candidate will participate in the advancement of patient care through interprofessional rounds, unitbased and organizational committees, quality improvement initiatives and practicebased research. The pharmacist will be required to provide occasional cross coverage and will work at both the Sunnybrook and CNIB sites to provide patient care.
