Responsibilities Assesses patients and collects information from a variety of sources; makes nursing diagnoses and determines appropriate nursing interventions. Collaborates with patients and families to develop care plans and encourages family participation. Participates in care and discharge planning, ensuring effective implementation and updates of care plans. Coordinates nursing care, sets priorities, assigns and delegates tasks to support staff, and makes referrals per protocols. Collaborates with interprofessional team members to identify and resolve issues and adjust care plans. Educates patients and families about illness prevention, health restoration, and maintenance. Initiates referrals to appropriate health care/community agencies. Documents assessments, diagnoses, interventions, patient/family responses, outcomes, and referrals. Participates in nursing committees, care conferences, and councils. Engages in research and quality improvement activities; identifies care issues and recommends changes. Attends inservices and educational programs; maintains updated clinical competence. Acts as a preceptor or mentor to new nursing staff and students. Provides feedback to management regarding patient care programs and equipment. Performs other related duties as required. Qualifications Education and Experience Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Skills and Abilities Complete initial and ongoing patient assessment and provide nursing care through appropriate interventions. Communicate effectively with patients, families, the public, medical staff, and interprofessional team members via verbal, written, and electronic means. Promote personcentered care with empathy, sensitivity to diverse cultures, and patient advocacy. Integrate and evaluate data from multiple sources to solve problems effectively. Teach patients and families about essential health topics. Organize and prioritize work. Collaborate and promote cooperation among interprofessional team members. Use factual information, prior learning, and evidencebased practice to support nursing decisions. Operate related equipment, including relevant computer applications. Possess the physical ability to perform the duties of the position.
