The nurse establishes and maintains relationships with, and consults with community health clinics, hospitals, housing providers and the interdisciplinary teams to provide appropriate treatment services based on best practices and client need. Program Summary ACT is an evidencebased transdisciplinary mental health team that provides outreach support to clients with severe and persistent mental health and substance use diagnoses. Qualifications Education and Experience: Bachelors or Diploma (RN/RPN) in Nursing Current practicing registration with the BCCNM (BC College of Nurses and Midwives) Three (3) years recent related experience providing treatment services to individuals with mental health and addiction concerns, or an equivalent combination of education, training and experience. Knowledge and Abilities Broad knowledge of diagnoses, treatment and support resources in the areas of mental illness and substance use. Knowledge and experience with harm reduction principles. Basic knowledge of legislation and government policies and procedures that relate to client, including the BC Mental Health Act. Demonstrated understanding of oppression and marginalization, specifically as related to people with experience of a mental illness, addiction, or homelessness. Ability to work with individuals from diverse backgrounds in an urban environment and willingness to provide flexible and creative care. Ability to work both independently and collaboratively as a member of a multidisciplinary team. Physical ability to perform the duties of outreach nursing, including light lifting (up to 15lb) and walking/stairs in community settings.
