Previous experience in the Mental Health and Addiction Services is an asset. A valid driver's license. Provides case management services, which includes assessing, planning, monitoring and evaluating for determining the care needs of clients, collaborative coordination of services and case management process. Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement. Position Responsibility Screening, assessment, and coordination of all initial service requests (written, telephone, walk-in, psychiatry and court ordered referrals.) to MH/AS and determining appropriate service according to client need. Responding promptly and effectively to initial requests for information and/or services, and planning other appropriate intervention or referral services for requests that are outside the mandate of MH/AS. To prioritize and assign requests for service to the most appropriate program/professional in a timely manner.
