Vancouver Coastal Health is looking for a Physiotherapist to join the Acquired Brain Injury (ABI) Transitional Housing Program team at the Oak Care Centre in Vancouver, BC. This innovative program is the first of its kind in BC, providing a healing, clientdriven transitional location for people after acute brain injury and hospitalization. As part of an interdisciplinary team, you will care for clients during transitions and support outreach in a traumainformed, harmreduction and culturally safe environment. Responsibilities Plan, deliver, evaluate and provide care coordination to address the needs of clients and families experiencing physical or functional impairment and acute/chronic disease within community, ambulatory and institutional settings. Work in a teambased environment in partnership with clients, family members or significant others. Utilize educational resources and collaborate with other health care professionals/providers to deliver direct care through the development, implementation and adaptation of a care plan. Provide care management to assist clients in managing their own health and navigating community services to address functional, physical, social, cognitive, perceptual and environmental needs. Contribute to program quality assurance initiatives, meetings, research and education. Education & Experience Current full registration as a Physiotherapist with the College of Health and Care Professionals of BC (CHCPBC). Eligibility for membership in the Physiotherapy Association of British Columbia (PABC). Valid BC drivers licence (personal vehicle required for local travel). Knowledge & Abilities Basic knowledge of evidencebased clinical practice related to the program. Basic knowledge of physiotherapy standardized tests and measures. Broad knowledge of physiotherapy theory and practice within a client and familycentred model of care. Broad knowledge of other health care disciplines and their role in client care. Effective teamwork, rapport building, ethical relationships and traumainformed practice. Cultural safety, Indigenous cultural safety and diversity competencies. Selfmanagement support, community resources and transitions through care continuums. Teaching, communication and independent work skills. Consultation, workload prioritisation and referral judgement. Basic research process knowledge and physical ability to perform duties. Proficiency with equipment and techniques appropriate to physiotherapy treatment. Hours and Policy The hours of work, including days off and work area, may be subject to change consistent with operational requirements and applicable statutes.
