OverviewBMP - Moose Jaw, Moose Jaw, SK S6H S6K, CANBayshore Medical Personnel is seeking a part-time Primary Care Nurse in Moose Jaw, Saskatchewan to provide service to our client in a primary healthcare setting.PCNs are responsible for the provision of primary health care services to members, and to provide patient assessment, screening, healthy lifestyle support, education, and chronic disease management with a goal of improving health outcomes and facilitating access to health services.They function as a member of a multi-disciplinary health care team to develop and implement care plans and provide patient follow-up.
