Job Description

OVERVIEW As an integral member of an inter-professional team, the Social Worker (SW) for the Mental Health Services Team cares for individuals with mental illness. The Social Worker will provide support in the Outpatient Mental Health team, supporting individual psychotherapy, psychoeducation, system navigation and group therapy. The Social Worker will support central intake and other clinics based on operational needs and for cross-coverage purposes In this role the SW conducts intake assessments on clients referred to the Outpatient Mental Health Program to determine appropriateness of service for the client. The Social Worker will also work closely with the interdisciplinary team to support patients seeking mental health support by conducting assessment, supporting stabilization and treatment/ follow up. The Social Worker will work with other clinicians in the Mental Health Services, East Toronto Health Partners (ETHP), Primary care providers and the community to connect client to services and safely discharge from the service.The Social Worker will help to support program development initiatives in the Mental Health and Substance Use Program. EDUCATION - Master of Social Work from an accredited school of social work, or equivalent degree, as recognized by the Ontario College of Registered Social Workers and Social Service Workers (OCSWSSW). - Must be a member of the OCSWSSW in good standing. - Certificates in psychotherapy modalities (e.g. CBT, DBT, MI) considered a strong asset. QUALIFICATIONS - A minimum of 5 years experience in a Mental Health/ Psychiatric/Substance Use services environment; Knowledge and experience in mental health assessments and intervention required. - Experience with group facilitation in a hospital-based setting required. - Demonstrated experience in intervention with high-risk populations. - Demonstrated excellence in crisis intervention clinical skills including mental health assessments, risk assessments, and safety plans required. - Demonstrated knowledge of the Mental Health Act, Mental Health Reform, Privacy and Consent to Treat Act and issues related to same required. - Demonstrated experience in applying the principles of crisis counselling in clinical practice required. - Demonstrated experience in developing community linkages and liaisons. - Demonstrated excellence in problem solving techniques, conflict resolution, and a willingness to contribution to high performing teams. - Good work and attendance record required. - All employees of MichaelGarron Hospital (MGH), a division of Toronto East Health Network (TEHN) formerly Toronto East General Hospital (TEGH)] agree to work within the legislated practices of the Occupational Health and Safety Act of Ontario. - All employees of MGH are responsible to contribute to a transparent culture of patient and staff safety by adhering to and abiding by patient and staff safety policies and procedures set by MGH. - All employees are accountable for protecting the psychological health and safety of themselves and their co-workers through adherence to MGH''s policies and practices.
