Job Summary We are seeking a compassionate Community Health Worker to support individuals and families by promoting health, wellness, and access to essential community services. This role focuses on outreach, client support, and coordination with healthcare and social service providers to improve quality of life for community members. Key Responsibilities Provide health education, outreach, and non-clinical support to clients Assist clients in navigating healthcare, social services, and community resources Coordinate appointments, referrals, and follow-up support Offer emotional support, advocacy, and companionship Collaborate with healthcare teams and community partners to ensure continuity of care Maintain accurate client records, documentation, and service notes Qualifications Experience in community outreach, social services, healthcare support, or non-profit work preferred Experience supporting seniors, vulnerable populations, or individuals with disabilities is an asset Strong communication, interpersonal, and organizational skills Compassionate, culturally sensitive, and client-focused approach Job Type: Full-time Pay: $23.00-$27.00 per hour Benefits: Extended health care Work Location: In person
