MSP uses housingfirst and harmreduction principles in the provision of a safe, respectful and accessible place for individuals who are experiencing homelessness or at risk of homelessness in the community. Since 1972, MSP has been providing a range of services to Winnipegs most marginalized residents, including emergency shelter and nutrition services, drug and alcohol withdrawal management services, health advocacy and support, case management services, housing support services, food bank services and a mobile community outreach programme. Main Street Project is committed to developing impactful and meaningful solutions that drive positive changes in our community. Job Summary The case management team is a key connection to outside organizations and participants accessing MSP services. Team members identify, develop, implement and assess participants selfidentified goals using a housingfirst, harmreduction, traumainformed lens. Through relationship building and coordination with primary care, income, addictions and mental health supports, case managers help participants secure and maintain housing. Key Responsibilities Provide ongoing case management support to assess progress and ensure treatment plan. Develop and coordinate individualized care plans addressing shortterm and longterm goals in collaboration with participants, interdisciplinary team members, and external partners. Assess an unsheltered persons needs and proactively identify barriers to housing, developing strategies to help participants meet goals. Create and maintain professional relationships with community members, including boundary setting. Keep accurate, continuous, legible and signed records including progress notes, checklists, correspondence, and journals for individual and collective access in accordance with established protocols for casework files. Work with involuntary participants and participants who cannot give consent. Apply casemanagement ethics to all participant relationships, including confidentiality, selfdetermination, informed consent and the power differential. Plan interventions with participants. Identify and provide information and resources to participants: Community resources Rent and income assistance Mental health resources Indigenous organisations Establish partnerships with landlords and service agencies, advocating on behalf of participants. Identify symptoms of suicide ideation and offer resources and support. Create environments of cultural safety for participants. Meet with participants both onsite and in the community. Monitor, evaluate and communicate progress of participants with themselves, team members and managers regularly. Maintain accurate, confidential electronic and file records, including statistical data. Conduct crisis intervention as needed. Attend programrelated community, coalition and committee meetings. Specialized Duties Outreach: Build effective relationships with encampment residents. Coordinate with local agencies to provide services that meet residents needs. Contribute to the development of a community that values cultural diversity. Preferred experience with homeless outreach, proven ability to access a wide range of homeless services, familiarity with overcoming barriers encountered by homeless people. Housing (The Bell): Provide supportive and proactive approach to eviction prevention. Create an open and welcoming environment for tenants to share housing concerns. Review client risks or issues and utilize a team approach to identify an action plan. Experience with palliative and afterdeath care. Experience with harmreduction in a supportive living environment. MMIWG2S: Provide comprehensive case management services to families affected by MMIWG, demonstrating sensitivity to cultural nuances. Maintain supportive relationships, acting as a liaison with law enforcement, social services and other community resources. Facilitate cultural ceremonies, healing circles, or support groups for affected families. Collaborate with local Indigenous communities, Elders, and cultural advisors to integrate cultural perspectives into practices. Contribute to data collection for research to inform systemic changes and improve support services. Qualifications Bachelors Degree in a relevant field; experience working frontline with unsheltered individuals, lived experience, and/or a combination of education and experience may substitute for a degree. Demonstrated knowledge of casemanagement principles and processes (eligibility, assessment, planning, implementation, evaluation, transition, documentation, file management). Experience developing and implementing care plans that promote positive health outcomes. Experience creating and maintaining case plans, progress notes and other necessary documentation. Experience working with Indigenous organisations and/or communities. Experience working with culturally diverse populations. Computer proficiency in Microsoft Office and any other database software used to track service delivery. Nonviolent crisis intervention (NVCI), mental health first aid, suicide intervention, and/or traumainformed training considered an asset. Knowledge, Skills and Abilities Strong knowledge of MSP Van Outreach work and relationships with unsheltered and encampment residents. Ability to apply a harmreduction and housingfirst approach to daytoday work. Understanding and ability to work within clientcentred, selfdetermination, strengthbased, and traumainformed frameworks. Excellent interpersonal skills and ability to form relationships with community members while maintaining professional boundaries. Motivational interviewing skills. Excellent verbal and written communication skills. Knowledge of Indigenous History, colonial history, and their impact in Manitoba. Knowledge of TRC Calls to Action, MMIWG Call to Justice, and UNDRIP. Indigenous language skills considered an asset. Knowledge of community resources across Manitoba. Knowledge of addictions, mental health diagnoses, and cooccurring disorders. Knowledge and ability to adhere to confidentiality standards and legislation. Ability to manage, assess, and work with challenging behaviours. Ability to work independently, under pressure, in a team or singleperson setting. Ability to effectively resolve and cope with conflict or crisis situations. Strong collaboration skills with team members and external partners. Ability to receive and apply feedback. Ability to model professional behaviours and demonstrate accountability with participants. Additional Requirement Satisfactory criminal record check with clear vulnerable sector search and a clear adult abuse registry check are required (employee responsible for service charges). Valid drivers licence. Current CPR and basic first aid. Conditions of Employment Ability to meet the physical demands, including standing for long periods, lifting up to 50lb, and performing cleaning duties. Exposure to biohazards (blood, bodily fluids, syringes). Ability to work with individuals who use substances and/or experience mental health issues. Ability to provide support in highintensity incidents (medical emergencies, aggressive individuals). Comfortable working in an environment where Indigenous ceremonies and practices occur (e.g., smudging). Comfortable conducting participants in the community, including encampments, hotels, public housing. Comfortable providing transportation for community members in company vehicle. Ability to occasionally flex hours to meet participant needs.
