Registered Practical Nurse (RPN) - Casual - Port Stanley Job Description: Reporting to the Director of Care (DOC), the Registered Practical Nurse (RPN) is responsible for providing planned, patientoriented, goaldirected nursing care in accordance with provincial standards of care. Key Responsibilities Work as a member of the interprofessional team as described by Extendicares Professional Practice Model, applying a personcentred approach to care. Complete and ensure resident clinical care assessments, care/service plans and documentation are accurate, uptodate and any changes are communicated as required. Implement and evaluate the effectiveness of care/service plans and make changes as required. Assist with scheduling resident care conferences and participate in them where applicable. Work closely with DOC to facilitate resident movein, moveout and transfers. Provide direct resident care and medication/treatment administration, documenting care in compliance with regulatory and Extendicare Resident Care standards and policies and procedures. Process orders, safely keep drugs and narcotics, and administer them in compliance with all regulatory requirements and Extendicare policies and procedures. Facilitate and provide resident/family education. Plan, coordinate, manage and evaluate the residents care plan working with the resident/substituted decision maker, physicians, nursing and interdisciplinary team and contracted service providers. Coordinate nursing team resident care assignments to ensure efficient and timely delivery of services to residents. Respond to and manage significant changes in resident condition or medical emergencies; provide emergency first aid. Teach, delegate to and direct unregulated care providers. Respond to resident/family concerns, ensuring appropriate action is taken and concerns are communicated to DOC/DHW/Executive Director and other departments. Follow and assist the DOC/DHW in the coordination, implementation and monitoring of Infection Prevention and Control, Health and Safety and Clinical Programs and Quality/Safety Plans for the Nursing Department. Uphold and promote the organizations values and philosophy regarding ethics, morality and integrity as set out in Extendicares Code of Conduct. Complete all other duties as assigned. Qualifications Current registration as a Registered Practical Nurse in the applicable province. Experience working in a senior's environment and with unregulated care providers (preferred). Education and experience in gerontology, rehabilitation, community/complex/longterm care (asset). Strong written and oral communication, critical thinking, care coordination, time management and customerfocus skills. Demonstrated sound clinical knowledge. Current Basic Cardiac Life Support and First Aid certification (required). Computer proficiency and experience with Microsoft Office applications.
