About Us The Hope to Health Clinic, Research and Innovation Centre in the Downtown Eastside is a unique and innovative health clinic comprised of a multidisciplinary clinical team focused on providing primary and specialty care for patients/ clients in the Downtown Eastside(DTES). The R&I centre is staffed by an interdisciplinary team (e.g., physicians, nurses, counsellors, peer navigators) and provides wrap-around healthcare to clients ensuring they receive the full continuum of care under one roof. Hope to Health Clinic works as part of the BC Centre for Excellence in HIV/AIDS.(BC-CfE). The BC-CfE is a world-renowned HIV/AIDS Centre with innovative, low barrier approach to healthcare delivery in clinical practice and an integrated group of research concentrations in Laboratory Sciences, Clinical Trials, Population Health and Epidemiology, Health Economics and Professional Education Programs. Job Summary Reporting to the Clinical Operations Manager and working in collaboration with the interdisciplinary team, the Licensed Practical Nurse is responsible for providing nursing care to an assigned group of patients/clients according to established policies and procedures and the Standards of Practice for the Licensed Practical Nurses of British Columbia. duties include participating in care conferences and in developing care plans, conducting assessments, implementing and evaluating interventions, documenting information, and acting as an advocate for patients/residents. What You Bring Education and Experience Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience. Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant. Current basic cardiac life support certification. One-year recent, related experience working in an Urban health setting including experience with addiction, HIV/AIDS or indigenous patient populations or an equivalent combination of education, training and experience. Skills and Abilities Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service. Ability to communicate effectively both verbally and in writing. Ability to deal with others effectively. Physical ability to carry out the duties of the position. Maintains communication with RN/RPN or other members of the interdisciplinary team as required. Uses evidencebased resources such as Nursing Care Standards and drug reference guides, inservices and workshops, to keep nursing francs up-to-date. Responds to a variety of incidents such as Codes, injuries or falls, hazardous spills, aggressive behavior and fire alarms.
