ConnectWell Community Health services are provided by salaried interdisciplinary team members working to their fullest possible scope, in a collaborative practice, to address the health and social well-being to residences, focusing on the social determinants of health needs of the people and communities we serve.The nurse practitioner provides primary health care to clients at ConnectWell Community Health. This ongoing position is located primarily at the Lanark site and Carleton Place satellite site locations. Apply today!Position Summary:The Primary Care Nurse Practitioner (NP) functions as a member of the Primary Care Team in the provision of primary health care services offered at ConnectWell Community Health. Primary health care services are provided at ConnectWell and in the community, with an emphasis on health promotion, client education and self-management. The RN (EC) must be aware of, understand, and adhere to the Standards of Practice set out by the College of Nurses of Ontario.Salary Range: $114,200 to $142,700 (1.0 FTE)Qualifications:Completion of recognized Primary Care Nurse Practitioner programMasters degree in Nursing, or equivalent recognized education and experienceCurrent and unrestricted registration as an RN (EC) with the College of Nurses of OntarioTwo years experience in the nurse practitioner role in a primary care setting or community preferredDemonstrated ability to work effectively in an inter-disciplinary teamDemonstrated leadership in promoting a shared responsibility for professional practice and developmentAbility to work evening hours, as requiredAbility to provide on-call telephone services on a rotation basisValid drivers license and use of a vehicleReporting Relationship: The successful candidate will report to our Senior Manager, Primary Care.HOW TO APPLY: Qualified candidates are invited to submit a cover letter and resume detailing their qualifications and interest in the position.
