The New Canadians Health Centre provides coordinated and multidisciplinary refugee-centered primary care utilizing a shared care model for a target population of government assisted and privately sponsored refugees as well as refugee claimants. Informed by refugees'' priorities that are communicated through culturally, linguistically, medically, and epidemiologically appropriate assessments, the supports create ready access to a comprehensive set of clinic and community services.PositionThe Nurse Practitioner (NP) will work collaboratively with the patients, family physicians, other team members to provide comprehensive care to patients of the New Canadians Health Centre. The NP will assess patients, as well as develop, implement, and evaluate care plans for patients both directly and indirectly. The NP will work to full scope of practice as an autonomous primary health care provider with an emphasis on providing evidence-based care, education, and guidance to patients with chronic disease, as well as supporting the successful transition of patients to community practices.Reports toClinical Services ManagerEducation/ExperienceMust have an active professional registration with the College Registered Nurse of Alberta (CRNA) as a Nurse Practitioner within the knowledge, skills and authority to provide care to Family-All Ages.Master''s or equivalent in NursingMinimum of three (3) years or more of related experience*Advanced skills in caring for patient with chronic, complex, and/or specialized needsCurrent BLS CPR is requiredProof of immunizations is requiredDiabetic Educator (CDE) would be considered an assetExperience in newcomer or refugee health would be considered an assetFluency in a second language, an asset*Nurse Practitioners who are either newly certified with less than three years of experience or will be graduating may be considered.SkillsBeing industrious, enthusiastic, adaptable, and having integrity are essential qualities for the Nurse Practitioner at the Edmonton North PCN.
