This part-time opportunity is to join our team at Central City Tower in Surrey, BC.ResponsibilitiesProvide individualized pharmacotherapy and client medication counsellingCollaborate with physicians and other health care providers to develop, implement and monitor therapeutic plansAssist in developing drug therapy guidelines for disease conditionsPrepare case studies and articles for professional journals; contribute to knowledge sharingAct as a department resource in the assigned areaParticipate in medication use evaluations, quality improvement initiatives and clinical drug trialsMentor and supervise pharmacy students, residents and new staffEnsure prescriptions are validated and dispensed accurately; address medication safety concerns with the health care team; document professional practice activitiesQualificationsCurrent registration as a Pharmacist with the College of Pharmacists of British ColumbiaCompletion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training and experienceBroad knowledge of organization policies, evidence-based clinical pharmacy practice, and medication use evaluationStrong communication skills and ability to work collaboratively with health care teamsAbility to organize, prioritize work and maintain accurate recordsPhysical ability to perform duties as requiredAbout Fraser HealthFraser Health is the heart of health care for over two million people in Metro Vancouver and the Fraser Valley in British Columbia.
