Job Description

Position Overview As a member of an interdisciplinary team, the Community Hospice Care Coordinator coordinates and administers the daytoday functioning of community hospice care services and programs. Working collaboratively with other Care Coordinators, this position manages a caseload of clients and caregivers and a team of volunteers to coordinate the administration of the InHome Support (Volunteer visits), Day Hospice and Caregiver Support programs. Community outreach and partnership building will be an integral part of this position. The schedule for this position will be Monday, Tuesday and Wednesday from 08:00AM to 04:30PM. This is an eight and a half (8.5) hour workday less an unpaid 30minute meal break daily. You also have two paid 15minute breaks to take at your discretion based on your workload. The payable hours of service will be eight (8) hours per shift. This position is being posted to fill a current vacancy. This position is eligible for group benefits and paid personal/sick leave.Key Responsibilities exportria> Manages a case load of clients and caregivers, maintaining client records and care plans. Manages and provides support to a team of volunteers. Makes home visits to assess appropriate clients and caregivers and refer to the Hospice Programs. Advocates on behalf of the patient, with their permission, by making appropriate phone calls to the attending physician, Ontario Health at Home Care Coordinator, andPrefered community resources. Liaises with volunteers regularly to support their work with clients and caregivers, and organizes quarterly volunteer meetings. Triage and process new hospice referrals in alignment with hospice eligibility criteria. Adheres to the quality standards of Hospice Care Ottawa and Hospice Palliative Care Ontario. Provide coverage for team members at all sites. Provides backup coverage on occasional weekends for leave of absences on a rotation basis. Supports Community Outreach and partnership building by attending to fairs, presenting on our programs/services and liaising with community partners. In-Home Hospice/Caregiver Support Program Provides intake to clients and caregivers calling from the community.Determines best suitability and assigns volunteers to clients/families for InHome Support visits. Act as patient advocate where needed and desired by the patient and family. Participates in patient conferences with other agencies and team members. Checks in with clients and caregivers on a regular basis, ensuring that their needs are being sufficiently met and in a way that is acceptable to them. Check in with volunteers on a regular basis to ensure they are well supported, and engaged. Day Hospice Program This position will run the Day Hospice Program at Maison de lEst. Assesses the needs of individual guests (clients) and performs client checkins and follow up as necessary. Coordinates the activities of the Day Hospice including scheduling of volunteers. Oversees and coordinates any complementary therapies e.g. music, art, massage, Reiki. Facilitates and ensures the smooth running of the Day Hospice. Arranges transportation via volunteer driver for those who require a drive. Qualifications Education Licenses and Certificates Bachelor of Science Nursing (BN) or equivalent. (Registered Practical Nursing designation may be considered with experience) Current Registration with the College of Nurses Ontario. Current CPR. LEAP Certification (Learning Essential Approaches to Palliative Care), or willingness to obtain within the first year of employment. Certification in Hospice Palliative Care Nursing Canada is an asset. Demonstrate a commitment to ongoing education and professional growth to maintain current knowledge in the changing face of both hospice palliative care and bereavement. Experience At least 3year experience in hospice palliative care and/or background in community palliative care nursing. Demonstrated experience in client assessment and patient advocacy. Experience with volunteer coordination, training and support an asset. Knowledge Understanding and demonstration of Ethics as they relate to Palliative Care. Strong knowledge of community resources. Understanding of the Francophone culture. Skills and Abilities Demonstrates a personfocused approach to service delivery.Demonstrates compassion, empathy, integrity, respect, courtesy and a caring attitude in all interactions. Bilingualism in both official languages is required. Superior communication skills oral and written French and English. Exceptional listening skills. Strong computer skills in Microsoft and experience in InfoAnywhere or similar medical software. Demonstrated ability to work as part of a work collaborative, interdisciplinary environment. Possess excellent organizational and time management skills and the demonstrated ability to handle multiple tasks and priorities. Other Must possess a valid Drivers License and access to a vehicle for the purposes of business travel. Must be able to work flexible working hours. Working Conditions Reporting Relationship: Reports to the Director, Francophone Services, & the Community Hospice Care Coordinator Team Lead. Position Status: Permanent, Parttime. Se conformer aux normes de qualit de la Maison de soins palliatifs dOttawa (MSPO) et de Hospice Palliative Care Ontario. Assurer la couverture des membres de lquipe aux autres sites, au besoin. Participer la rotation pour la couverture des absences. Programmes de soutien domicile et programme de soutien la famille Accueillir et orienter les appels des clients et de leurs proches de la communaut. valuer les aptitudes des bnvoles et les jumeler aux clients pour les visites de soutien domicile. Programme de soins palliatifs de jour Ce poste consiste grer le programme de soins palliatifs de jour la Maison de lEst. valuer les besoins des invits et effectuer le suivi. Coord Paperback les activits du programme, y compris lhoraire des bnvoles. Superviser et coordonner les thrapies complmentaires (musique,UBLISHED, arts, massages, Reiki). Assurer le bon fonctionnement du programme de soins palliatifs de jour. Organiser le transport via un chauffeur bnvole pour ceux qui ont besoin dun lecteur baf. Qualifications requises Formation, inscription et certificats Baccalaurat en sciences infirmires ou quivalent. Membre en rgle lOrdre des infirmires et infirmiers de lOntario titre dinfirm proclamation. Certificat de RCR jour. Certification LEAP (Learning Essential Approches to Palliative Care) ou volont de lobtenir dans la premire anne demploi. Certification de lAssociation canadienne des soins infirmiers palliatifs (atout). Formation en planification pralable des soins (Canada). Engagement envers la formation continue et le perfectionnement professionnel. Exprience Minimum de trois (3) ans dexprience en soins palliatifs, incluant les infirmiers palliatifs en milieu communautaire. Exprience dans lvaluation clinique des clients et la dfense des intrts des clients. Exprience dans la coordination, la formation et le soutien aux bnvoles Connaissances Comprhension approfondie de lthique des soins palliatifs. Connaissance des ressources communautaires pertinentes. Bonne comprhension de la culture francophone. Comptences Approche centre sur la personne. Compassion, empathie, intgrit, respect et bienveillance de courtoisie dans toutes les interactions. B'intrieurdnomination obligatoire (franais et anglais). Excellente comptences en communication orale et crite dans les deux langues officielles. Excellente capacit d'coute.
