Pay Range Rate: $25.75$31.50/visit (placement on salary range as per Collective Agreement) Estimated annual earning potential: $70,000$120,000. Actual earnings may vary based on availability and service volumes Role Overview Grounded in our commitment to Client and Family Centred Care the key foci on the RPN role is to provide and coordinate home health nursing services to a diverse group of individuals, families and groups according to the College of Nurses of Ontario Practice Standards, the policies and procedures of VHA, the program where they provide services, and the Standards of Practice in Community Health Nursing. We are seeking Registered Practical Nurses with Advanced Foot Care Certification to work as Casual Nurses in the Central and Central West regions. This program focuses on providing foot care services to clients in community clinics, longterm care facilities, and homebased settings. Seniors and diabetics who may have challenges caring for their feet can receive regular visits from our certified foot care nurses to improve their overall health and mobility. This role involves taking care of clients feet with a focus on prevention, treatment and ongoing care management to keep their feet happy and healthy. This is a visiting role, and a valid G drivers license and vehicle are required. Key Responsibilities Assess, monitor, and make care planning and coordination Evaluate care plan interventions through ongoing reassessment and evaluation results Ensure the quality of care of patients by identifying and acting on factors which affect practice autonomy and delivery of quality care Demonstrate professionalism, leadership, judgment, and accountability in interactions with clients and families Support safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement Inform nursing supervisor of any client activities and relevant concerns or changes in the client or the client care plan, and submit appropriate documentation to the case coordinator Maintain documentation standards according to the College of Nurses of Ontario Required Qualifications Registered and in good standing with the College of Nurses of Ontario (CNO) RPN diploma from a recognized university or college Membership of your Professional Association preferred (RNAO/WeRPN) Have a valid Advanced Foot Care Certificate from a publicfunded college or university Foot Care Certification from Ruth Ruttan & Assoc. is accepted Worked a minimum of 100 hours as a Foot Care Nurse Valid First Aid or CPR Certification Acute Care and MedicalSurgical nursing experience is an asset Knowledge of community nursing is an asset Demonstrate effective communication (written and verbal) and documentation skills Demonstrate reliable attendance, punctuality, and availability for the role Commitment to modelling lifelong learning and professional growth and can apply critical thinking skills to provide client care A valid Ontario drivers license (G License) and a reliable vehicle for traveling is required What makes VHAs compensation unique?
