Department/Program/Unit Summary: The Inpatient Medicine Program consists of a more than 350 budgeted beds across the CVH & MH and is staffed by inter-professional teams who support the care of critically ill and complex medicine patients within the Mississauga-Halton LHIN. Our teams provide high quality-patient centered care in an innovative environment to support the physical, mental, emotional and spiritual needs of our patients.We are dedicated to building a diverse workforce that reflects the community in which we live and serve. Position Summary: Reporting to the Director of the Inpatient Medicine Program, the Clinical Nurse Specialist (CNS)is an advanced practice nurse whose role iscomprisedof five domains of practice including clinical practice, leadership, collaboration,researchand consultation. TheCNSwill support the Inpatient Medicine program by performingpatient assessments, providing patient education,supporting complex patient and family situations,and coordinating care for patients withCongestive Heart Failure (CHF)in the emergency department, inpatient medicine settings, and our ambulatory programs. In theinitialphase of this role, theCNSwill work collaboratively with the CHF project group to develop and implement practice improvements and changes (including optimization of the role of theCNSin the care of patients with CHF) that will positivelyimpactthis patient population at THP and beyond As the project transitions to implementation,theCNSwill shift focus to direct patient care, working in a collaborative practice model with physicians, nurses, inter-professional teammembersand community partners to provide quality care to patients and their families. TheCNSwillalsosupport CHF pathway implementation by engaging in staff education, mentorship,andchangemanagement. TheCNSwill provide a key leadership role in evaluating quality of care and patient outcomes within the CHF population, in partnership with physicians and other members of the inter-professional team. TheCNSwillprovide leadership in the critical analysis and review of literature and the application of research findings to practice through the development of Clinical Pathways and implementation of best practices. TheCNSwill provide leadership andassistwith professional development of team members through mentoring andparticipatingin formal and informal education initiatives. Qualifications: ***Please note that a Master of Nursing or Master of Science in Nursing is required for this role*** Registered Nurse with Ontario Certificate of Competence with the College of Nurses of Ontario MasterofNursing or Master of Science in Nursing Minimum 5 years of relevant,related experience as a Registered Nurserequired,Med-SurgorCardiologypreferred BCLSrequired Experience working/leading with interdisciplinary teams Experience in Adult education Clinical specialty organizational membership: RNAO (required) Demonstrated excellent communication and interpersonal skills, with the ability to workindependentlycollaboratively with physicians, inter-professional team members, and other stakeholders Possesses highly developed analytical, problem solving and critical thinking skills Excellent oral, written, and presentation skills, withdemonstratedcompetency in information and communication technology Demonstrated excellent organizational and coordination skills and the ability to prioritize multiple competingworkloaddemands Demonstrates an ability to provide leadership,supportand critical analysis in stressful, complex clinical and emotional situations Internal Candidates who believe they possess the necessary qualifications and experience for this position and who have been in their current position for at least six (6) months are encouraged to apply.
