Description: Under the supervision of the Director, Medical Homes and Projects, the Primary Care Nurse provides comprehensive nursing services within the patient''s medical home. As part of a collaborative, inter-professional team, the nurse assists patients through all stages of life. The nurse will work with family physicians in the medical home & supports various degrees of wellness and illness by providing assessment, screening, healthy lifestyle support, education & chronic disease management, with the goal of improving health outcomes and facilitating access to services. QUALIFICATIONS: Education and Certifications: Graduate of an approved Canadian License Practice Nurse (LPN) program Current licensure with College of Licensed Practical Nurses Current Basic Life Support (BLS) for healthcare provider Must have own vehicle and valid driver''s license Experience: Primary care and/or at least 5 years nursing experience Caring for patients with chronic, complex, and/or specialized needs Health assessment and screening to identify patient health needs Performing primary care procedures (but not limited to) wound care medication and immunization administration, cryotherapy, ear syringing, and women''s health screening (PAPs) Working with and assisting physicians in procedures Providing basic healthy lifestyle education for patients, including healthy behaviours, risk reduction and illness prevention Assisting and supporting patient navigation through the healthcare system and community Proficiency with electronic medical records is an asset. Skills and Attributes: Excellent time management and organizational skills Excellent interpersonal and communication skills Able and willing, in an unstructured environment, to be flexible, creative and engage in collaborative relationships with health care providers Ability to act responsibly and be accountable for actions Ability to work autonomously, independently and to prioritize work activities Requirements: A current and clear Police Information/Vulnerable Sector check, and a record of immunization history.
