Job Description

Overview The Yukon Home Care Program provides client-focused health services in the home, improving quality of life and supporting Yukoners to live independently. Home Care offers a range of services tailored to meet individual needs, including support for acute care, chronic and palliative conditions. These services are determined through standardized assessments to assist in creating individualized plans of care; and are supported by an interdisciplinary team, consisting of Physiotherapists, Occupational Therapists, Registered Nurses, and Home Support Workers. Responsibilities Coordinate home care services in Yukon communities outside of Whitehorse. Supervise home support workers in rural and remote communities. Coordinate discharges from hospital and complete assessment for longterm care. Perform case management and collaborate with community partners. Qualifications A masters degree in Physiotherapy or Occupational Therapy. Current registration or eligible for registration with a Canadian Occupational Therapy regulatory body or current registration as a Physiotherapist in the Yukon obtained through The Professional Licensing and Regulatory Affairs Branch (Community Services). Experience planning, developing and implementing programs. Experience supervising support staff. Experience as a Physiotherapist or Occupational Therapist providing assessments and treatments in a community environment (asset). Strong ability to collaborate and work within a multidisciplinary team. Strong skills working with various cultural backgrounds. Excellent organizational, interpersonal and conflict management skills. Effective oral and written communication skills. Ability to work independently within a team setting. Ability to supervise and provide leadership for support staff. Strong assessment skills. Conditions of Employment TB screening. RCMP Security Clearance with Vulnerable Sector Check. CPRC with AED (within last 30 months). WHMIS 2015. Valid Class 5 drivers license. Current registration with a Canadian Occupational Therapy regulatory body or a valid current license to practice physiotherapy in the Yukon obtained through The Professional Licensing & Regulatory Affairs Branch (Community Services). Job Requirements / Exposure Travel within the territory by road and air (sometimes during inclement weather conditions). Exposure to pets, secondhand smoke, latex products, flowers, and/or plants and other irritants while in client homes. Exposure to infectious agents.
