POSITION OVERVIEW As the Clinical Lead RN, you play a key role in team-based primary care, supporting continuity of care, early intervention, and preventative health strategies in collaboration with physicians, nurse practitioners, mental health professionals, and allied health providers. In addition to the scope of an RNs practice and authorized services, the Clinical Lead Registered Nurse will fulfill a clinical leadership capacity providing clinical supervision and mentorship to regulated health professionals on the team as well as clinical quality oversight. Position: Registered Nurse Primary working location: Covenant Family Health Clinic, Community Wellness Centre - Lakewood FTE: 1.00 This is a Full Time position of 40.0 hours per week Minimum Salary: $41.16 Maximum Salary: $54.34 KEY RESPONSIBILITIES: Clinical Oversight: You will provide clinical orientation to care providers, carry out performance development for care providers, design/initiate clinical standards and process development, lead clinical quality assurance management, provide clinical education and professional development leadership. Care Coordination & Clinical Triage : Coordinate referrals and follow-up care. Provide clinical triage and decision support patient care. Assessment & Planning : Conduct nursing assessments and triage. Develop and update individualized care plans. Collaborate with interdisciplinary teams to support patient care. Intervention & Support: Participate in shared decision-making with patients and caregivers. Administer medications and treatments. Utilize validated screening tools (e.g., pain scales, frailty index, PHQ-9) to inform care planning. Provide wound care, immunizations, and chronic disease support. Respond to urgent care needs and clinical deterioration. Health Promotion : Deliver health education and self-management support. Facilitate wellness clinics and preventative care initiatives. Documentation & Communication: Maintain accurate clinical records. Communicate effectively with patients and team members. You are able to self-regulate and thrive in a possibly high-stress work environment. WHAT YOU BRING TO THE TABLE: Education: Bachelors degree in nursing. Current registration with CARNA. Experience: Minimum of five (5) years progressively responsible related experience in a registered nurse role, four (4) of which are in a primary care, clinical or community setting. Equivalent education and experience may be considered.
