Company Description Community Care Concepts is a non-profit volunteer-based organization that provides high quality support services throughout the Townships of Woolwich, Wellesley and Wilmot that enable seniors and adults with disabilities to remain as independent as possible in their own homes. Role Description We are searching for an exceptional individual to work in the community as a Community Navigator to fill an existing vacancy in a full-time, approximately 6-month contract , with the possibility of extension based on funding and program needs Working collaboratively with members of our health system, clients and caregivers, the Community Navigator will identify and assess client needs and develop, implement and monitor service plans that address identified needs. This is an in-person role based in Waterloo Region. The salary range for this position is $35,700 to $43,197 annually. Qualifications The ideal candidate will possess a diploma or degree in the Social Sciences, Health or a related discipline accompanied by 3-5 years of experience working with older persons or adults with disabilities.Well-developed intake, assessment, referral, crisis assessment, intervention, counselling and case management skills are essential. The successful candidate must be willing and able to travel throughout the Region of Waterloo to support clients and collaborate with health teams in various primary care offices . This role requires flexibility to work in different community and clinical settings based on need, with a strong focus on building relationships and ensuring continuity of care across the region. Computer proficiency as well as strong oral and written communication skills, an ability to work independently, program planning and delivery skills and knowledge of the needs of the client base are essential.A valid drivers license and access to a reliable vehicle on a daily basis are essential. A mandatory vaccination policy is in place within the organization.
