The Registered Nurse (RN), Certified Diabetes Educator (CDE) within the Diabetes Education Program in the St. Michaels Hospital Academic Family Health Team (SMHAFHT) is a highly motivated, self-directed member who advocates for wellness and delivers safe, effective, and ethical nursing care. This role provides comprehensive diabetes education and management for individuals across the lifespan and supports health promotion and disease prevention initiatives to optimize health for patients and families. The RN CDE will deliver direct care to individuals living with diabetes and its complications and will develop and deliver group education. The CDE collaborates with the interprofessional team to advance the SMHFHT approach to diabetes care. The primary location of this position is across all five Family Health Team sites as needed. This role requires coverage for regular day hours 95 and occasional evenings 128. Locations: 61 Queen St. East; 80 Bond Street; 140 The Esplanade; 95 Homewood Ave; 73 Regent Park Blvd. Responsibilities The primary role of the RN CDE is to provide holistic, evidence-based, culturally appropriate, patient-centered quality care in accordance with the College of Nurses of Ontario standards and Unity Health Torontos mission, vision, and policies. The RN CDE optimizes the patient experience and care outcomes by: Being accountable for ones decisions and professional practice. Establishing a therapeutic relationship with patients. Developing group education materials, including educational sessions and patient handouts, in an equity-focused manner. Collaborating with other members of the patient care team. Coordinating and communicating patient care activities to optimize the patients experience and care outcomes; and Collaborating and acting as a resource for physicians, nurses and other interprofessional team members in the care of patients with diabetes. Responsible for collecting and reporting data quarterly and as needed to monitor program effectiveness and impact. Patient Care Area Of Responsibility Accountabilities Acquires information to assess patient care and learning needs. Engages patients and families to assess involvement in care and integrates it into care planning. Develops a plan of care and integrates nursing activities with other inter-professional activities. Engages in critical inquiry and reflection to select evidence-based interventions. Organizes nursing care based on patient care needs and priorities. Provides ongoing care to clients of all ages, including assessments, counseling, screening, referrals, education, and follow-up within a primary care framework and guided by the RNAO Best Practice Guidelines. Initiates and co-manages appropriate nursing interventions and uses current knowledge trends to care for patients and/or monitor ongoing therapy. Promotes self-management and behavior change to address chronic diseases early in the disease cycle to prevent progression and complications. Collaborates with the client, family, and interprofessional team to develop health care plans and recommendations concerning treatment, prevention, and health promotion options. Anticipates changing situations; manages transitions from stable to unstable, establishes priorities, and collaborates with the care team as needed. Provides health education using effective teaching strategies that consider learning needs, readiness, and motivation to learn. Collaborates with appropriate team members on patient care needs. Processes orders and seeks verification from prescribers. Contributes to activities to facilitate care continuity and coordination; advocates for care needs to be met in the most appropriate setting. Qualifications Include Education/Certifications Certified Diabetes Educator (CDE) certification required Current BLS Certificate Complete Course on Prevention and Management of Aggressive Behavior (Crisis Intervention) within 6 months Professional Registration Current General Class (Practising) Registration with the College of Nurses of Ontario Required Experience Minimum 23 years of clinical experience working with patients and families living with diabetes (Type 1 or Type 2 requiring insulin) and its complications Required Skills/Competencies Experience in group program development, implementation, monitoring, and evaluation Knowledge of adult learning principles Experience in a community primary care or ambulatory setting, preferably in an underserviced area, or in combination with community and hospital/public health settings Commitment to primary health care with a focus on health promotion, disease prevention, and chronic disease management Ability to work in an interprofessional team Experience in teaching and providing health education to individuals and groups, and to interprofessional learners Excellent computer skills and at least 2 years experience with an EMR in a Primary Care setting or equivalent Knowledge of current evidence-based methods of primary care delivery Excellent interpersonal, communication, decision-making, problem-solving, conflict management, and time management skills Satisfactory attendance record Unity Health Toronto is committed to accessibility and inclusion.
