The program is one of seven sites across the city which offers a dynamic environment for healthcare leaders. Raven Song Primary Care Clinic, located at 2450 Ontario Street in Vancouver, provides primary care services to residents of Vancouver who are living with complex clinical and psychosocial needs; are often vulnerable and underserved, and require a higher intensity of services to achieve and maintain functional stability. Clients are approached with a view to support the transition to less intense service settings if stability is gained. This position combines a 0.4FTE posting at St. Pauls Hospital Department of Psychiatry for a total of 0.9FTE. Responsibilities Reporting to the Medical Director, the physician who accepts this position will play a critical role in supporting the ongoing primary care needs and in the development of an interprofessional shared care team. The responsibilities include: Providing direct clinical services to patients who present or are referred for care. Fostering a collaborative interprofessional practice environment that enhances clientcentered services within the associated clinics/sites/programs. Working within a selfdirected team of physicians and other interprofessional colleagues to achieve excellence in clientcentered care.
