Job Summary We are seeking a compassionate and dedicated Care Manager to join our team at Opulife. This role is vital in overseeing the daily operations of our care team, ensuring exceptional service for clients and their families. The ideal candidate will have a background in nursing, strong leadership abilities, and a passion for providing high-quality, person-centered care. This position combines care management, staff supervision, recruitment, and operational oversight to ensure that our clients receive the best possible support. Duties Develop and implement individualized care plans tailored to each client''s unique needs. Conduct assessments to evaluate clients'' physical, emotional, and social requirements. Coordinate services such as medication administration, pain management, and dementia care. Monitor and evaluate the quality of care to ensure alignment with client needs and expectations. Supervise, mentor, and support care staff, fostering a collaborative and compassionate work environment. Oversee recruitment, onboarding, and training of new staff members. Ensure compliance with care plans, quality standards, and regulatory requirements. Manage scheduling, office administration, and the coordination of care services. Maintain detailed records of client progress, interventions, and outcomes. Collaborate with healthcare professionals and families to ensure seamless communication and care continuity. Act as the primary point of contact for clients and their families, ensuring open and empathetic communication. Provide education and resources to families regarding health conditions, treatments, and care options. Stay current on best practices in senior care, assisted living, and home care services. Innovate and improve processes to uphold Opulife''s high standards of care. Qualifications Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College. Minimum 1-2 years of leadership / managerial experience in healthcare, preferably in home care services. Proven experience in care planning, staff supervision, and team management. Knowledge of pain management techniques, medication administration, and dementia care. Strong interpersonal and communication skills with the ability to build relationships and engage key stakeholders. Excellent organizational skills with the ability to manage multiple tasks simultaneously. Familiarity with medical terminology relevant to senior care. Valid driver''s license and reliable transportation. Valid Vulnerable Sector Criminal Check for clearance to work with vulnerable populations. Updated immunization record, including a TB test (within the last 6 months). Fluency in Italian is an asset but not mandatory.
