Job Title: Nurse Practitioner Total Hires: 3 Location: Toronto, ON Schedule: Full-Time (Contract) Weekdays: Monday to Friday Hourly Rate of Pay: $100 - $120 Carecor is seeking compassionate Nurse Practitioners (NPs) to deliver virtual and mobile substanceuse care to individuals experiencing homelessness in Toronto. Using a harmreduction and clientcentered approach, NPs provide assessment, treatment, and crisis support for clients with complex substanceuse disorders, including opioid, alcohol, and stimulant use disorders. Summary of Position: The Substance Use Nurse Practitioner (NP) works collaboratively with nurses, physicians, shelter case managers, community workers, and agency staff to deliver substanceuse care and limited primary care to clients experiencing homelessness, in alignment with PHIPA and client consent requirements. The NP provides comprehensive, advanced assessmentssupported by the mobile substanceuse nurseto address both urgent and ongoing substanceuse needs, escalating concerns to a physician or Emergency Services when appropriate. Working closely with shelter and city partners, the NP also offers social and emotional support, including crisis mitigation and case management. The role receives indirect supervision and ongoing guidance from a Nurse Manager. Key Responsibilities Conduct comprehensive assessments, diagnose, and develop clientcentered care plans using a harmreduction approach. Provide accessible virtual/mobile substanceuse care, including evidencebased treatments and pharmacotherapy (e.g., OAT, anticraving medications, withdrawal management). Address social and structural determinants of health impacting client wellbeing. Collaborate with physicians, nurses, shelter staff, and community partners to ensure coordinated care and connect clients to longerterm support. Maintain accurate documentation using the EMR system and support policy, guideline, and medical directive development. Contribute to nursing team knowledge through training and support initiatives. Perform additional nursing duties as assigned by the Nurse Manager. Current registration as an NP (EC) in good standing with the College of Nurses of Ontario; Master''s degree required. Liability/malpractice insurance, current BLS certification, and a valid vulnerable sector police check. Minimum 3 years of nursing experience preferred; experience in substance use, primary care, street outreach, or community health is an asset. Demonstrated expertise in harmreduction approaches, mental health, and substanceuse treatments. Commitment to antioppressive and antiracism principles. Additional training in addictions or controlled substances (e.g., CAMH, OAT) considered an asset. Strong clinical judgment, communication, organizational, and interpersonal skills; ability to work effectively on an interdisciplinary team.
