As an LPN/RPN, you are passionate about Making Lives Better and promoting Happier Aging. You are committed to providing peace of mind to both clients and their families, and you work closely with your Care Designer to deliver a premium care experience while meeting client care goals. Duties and Responsibilities Demonstrate excellent customer service skills and interact professionally and tactfully with clients and interdisciplinary team members Act as a brand ambassador by treating clients and team members with respect and courtesy Review care plans prepared by the Care Designer prior to each visit, accurately complete documentation, and carry out assigned duties as directed Respond to Care Services Center (CSC) scheduling inquiries and updates within agreed timelines Maintain open communication with the Care Designer when opportunities to enhance client care or experience are identified Provide comprehensive professional nursing care aligned with care plans, nursing assessments, and client needs Perform all nursing skills, treatments, and procedures competently in accordance with provincial LPN/RPN scope of practice and Nurse Next Door policies, procedures, and protocols Assess clients and provide education and information to clients and families/caregivers related to diagnoses and care needs Assist clients with activities of daily living, including hygiene care, bathing, grooming, linen changes, and related support Requirements and Experience Current Certificate of Competence and good standing with the provincial College of Nurses Graduation from a recognized Practical Nursing program with at least one year of recent gerontological nursing experience Ability to practice safely and competently within personal knowledge, experience, and job scope Minimum of one year of nursing experience required; three years preferred Community, home health, or medical/surgical experience is an asset Criminal Record Check with Vulnerable Sector Search (renewed at the nurse''s expense every five years for continued employment) Negative TB test or chest X-ray (renewed at the nurse''s expense every five years for continued employment) Reliable access to a vehicle for traveling to and from clients'' homes Availability Flexible availability required Casual, part-time, and full-time opportunities available Business Relationships Reports to the Care Designer Works closely with the Client Care Ambassador Key Metrics Attendance and reliability AMS Attendance Monitoring System (AlayaCare) Job Types: Full-time, Part-time Application question(s): Do you have access to a personal car Experience: supportive care: 1 year (required) Language: English (required) Filipino (preferred) Cantonese (preferred) Mandarin (preferred) Khmer (preferred) Thai (preferred) Licence/Certification: Criminal Record/Vulnerable Screening Sector ( Negative TB Skin Test or Chest X Ray ( Registered Care Aide or Nursing Certification (required) First Aid & CPR Certification (required) Driver''s license (required) Willingness to travel: 100% (required) Work Location: In person
