Your Opportunity The Registered Nurse (RN) Case Manager collaborates, coordinates, and delivers healthcare services, with expertise in advocacy and a patient and familycentered approach. You will work with clients across the life span with various needs in the home setting. You must be willing to drive up to one hour from your home site to the clients residence during all seasons and at various times of the day. This position does have some physical requirements such as bending, lifting, twisting, walking, climbing stairs, and carrying the lifting and carrying could include heavy items. This position requires oncall duties. Responsibilities include assessing, planning, implementing, and evaluating client care, focusing on chronic disease management, health assessments, wound care, ADL support, placement in facilities, and palliative care. The RN Case Manager collaborates with individuals, caregivers, and community resources to support healthy living and community connectedness. Located in beautiful Fort McMurray which resides on Treaty 8 territory and on the traditional lands of the Metis. Each Indigenous community is diverse in size, culture, and character. In addition to a competitive rate of pay, ALA currently has a Fort McMurray Allowance in place to an annual maximum of $12,480. This allowance is nonpensionable and is payable on an hourly basis for all hours paid at the basic rate of pay. Description As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and wellbeing. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58$54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support Health Care Provider (BCLSHCP) certification. If you have not practiced as a Registered Nurse within the past five (5) years, completion of the following free courses is required prior to employment: CRNA Medication Management (Chapters 14): NextGenU Health Assessment Resources Certification: Additional Required Qualifications Bedside nursing of 2 years in the last 5 years. You will need to supply a reliable vehicle and carry business insurance on your vehicle, which will be reimbursed for. You must be comfortable driving roads and at times, that drive, could be in the middle of the night, in winter conditions if needed. Ability to be on call as required. Inter RAI HC certification. Connect Care experience. Experience with assignment of care, and supervision of unregulated health care staff. LEAP certification. Experience in wound vac, wound care, ostomy and continence care. Preferred Qualifications AADL (Aids to Daily Living) authorization certification preferred. 13 years of Home Care Case Management and wound care experience. #J-18808-Ljbffr
