Job Summary We are seeking a compassionate and dedicated Registered Practical Nurse (RPN) to join our healthcare team. Duties Administer medications and treatments accurately following physician orders and established protocols Monitor patient vital signs and assess health status regularly Assist with patient mobility, hygiene, and comfort needs Document patient care activities using electronic health records such as Epic or Cerner systems Collaborate with interdisciplinary teams to develop and implement individualized care plans Provide specialized care for patients with dementia, hospice needs, or acute conditions Perform phlebotomy procedures and laboratory specimen collection as required Educate patients and families about health conditions, medication management, and post-discharge care instructions Support case management activities to coordinate ongoing patient services and resources Ensure compliance with infection control protocols and safety standards in all aspects of care delivery Experience Prior experience working in nursing homes, hospitals, assisted living facilities, or home care settings is preferred Knowledge of medical terminology, medication administration, pain management, and patient safety protocols is essential Experience with electronic medical records systems such as Epic or Cerner is advantageous Background in pediatric care, senior care, hospice support, or dementia-specific programs is desirable Demonstrated ability to handle complex cases involving acute care needs or chronic conditions Strong communication skills and the ability to work effectively within multidisciplinary teams Certification as a Registered Practical Nurse (RPN) is required This role offers an enriching opportunity for nurses committed to providing exceptional patient-centered care across diverse healthcare environments.
