Job Title: Licensed Practical Nurse (LPN) Care Leader Position Summary The LPN Care Leader plays a critical leadership role in delivering high-quality, client-centered care within the community. This position is responsible for conducting nursing assessments and care consults, completing client visits, developing and updating care plans, and providing education, training, and oversight to caregiving staff. In addition to clinical leadership, the Care Leader supports community engagement and sustainable business growth by building trusted relationships with clients, families, referral partners, and community stakeholders. Through professional representation, consultative conversations, and participation in community and company events, the Care Leader contributes to service awareness, client growth, and long-term partnerships. The Care Leader partners closely with operations, scheduling, and business development team to ensure exceptional client outcomes, caregiver performance, continuity of care, and service excellence. All duties are performed within the scope of practice of a Licensed Practical Nurse, in accordance with CLPNBC standards, employer policies, and applicable legislation. Key Responsibilities 1. Client Assessments & Care Consultations Conduct comprehensive nursing assessments and in-home care consults for prospective and existing clients Educate clients and families on care options, services, and individualized care solutions Develop care plans aligned with client goals, needs, and budget Participate in follow-up visits and reassessments as client conditions change Support client growth through relationship building, consultative discussions, and appropriate service recommendations 2. Client Visits & Ongoing Care Oversight Attend initial client visits to establish care standards and ensure safe service initiation Complete supervisory and quality assurance visits Monitor client health status and satisfaction, updating care plans as required Ensure care delivery aligns with approved care plans and organizational standards Maintain accurate and timely documentation in CRM software and G-Suite platforms 3. Education, Training & Staff Development Provide clinical education, mentorship, and coaching to caregivers Support onboarding, skills refreshers, and ongoing training initiatives Reinforce best practices in medication support, personal care, safety, and documentation Address performance concerns and support professional growth of caregiving staff Promote a culture of accountability, compassion, and client-centered care 4. Caregiver Leadership & Support Lead and support caregivers to deliver consistent, high-quality care Foster strong caregiver engagement, retention, and job satisfaction Support appropriate caregiver-client matching Communicate care plan changes clearly and promptly to caregivers 5. Collaboration with Scheduling & Operations Liaise closely with scheduling and care support teams to ensure adequate coverage Assist with resolving scheduling gaps and urgent care needs Communicate changes in client condition or care requirements Support operational efficiency and continuity of care 6. Community Engagement & Business Development Conduct visits to independent living and assisted living facilities as required Build and maintain positive professional relationships with community partners, referral sources, and stakeholders Participate in community outreach initiatives that support brand awareness and client growth Support consultative discussions that identify client needs and appropriate care solutions Attend and represent the organization at company events, including conferences, trade shows, community events, and professional networking opportunities Collaborate with internal sales and marketing teams to support referral development and community-based initiatives Represent the organization with professionalism, integrity, and a client-centered approach at all times Scope of Practice All responsibilities are performed within the authorized scope of practice of a Licensed Practical Nurse, as defined by CLPNBC standards, employer policies, and applicable legislation Required Qualifications Graduation from a recognized Practical Nursing program Current full practicing licensure in good standing with the College of Licensed Practical Nurses of BC (CLPNBC) Minimum 1 year of recent experience in gerontological, community, or home care nursing CPR and First Aid Certification Negative TB test (updated every 5 years) Criminal Record Check with Vulnerable Sector Search (updated every 5 years) Valid BC driver''s license and reliable vehicle Legal authorization to work in Canada Skills & Competencies Strong clinical assessment and care planning skills Excellent written and verbal communication skills Proficient computer skills (email; care management software experience an asset) Strong organization, time management, and multitasking abilities Goal-oriented with the ability to track progress and outcomes Client-focused, solution-oriented approach Ability to work independently while collaborating effectively with multidisciplinary teams Assets / Nice to Have Experience leading or mentoring care teams Home care, case management, or community nursing background Experience with consult-based client onboarding or service conversion Customer service, relationship management, or leadership experience Job Types: Part-time, Casual Pay: $32.00-$40.00 per hour Expected hours: 15 40 per week Benefits: Company events Extended health care Flexible schedule Mileage reimbursement Application question(s): Do you have driver''s license and access to a vehicle?
