About The Aspen PCN The Aspen Primary Care Network first became operational in 2010, serving three communities. Since then, we have grown to include partnership between family physicians in Athabasca, Westlock, Barrhead, Swan Hills, Kinuso, High Prairie and Wabasca, Alberta. In total, the PCN has 43 participating physicians serving approximately 30,000 residents within our region. Our priorities are to facilitate health promotion and disease prevention, and to enable successful self-management of chronic diseases and conditions. This position is with the Aspen PCN FTE: 0.4 Status: Temporary 16 months from start date Days per Week: 2 Hours per Shift: Reference ID: FPNW20260123 Primary Location: Westlock, AB Position Summary The Aspen Primary Care Network is currently seeking a Licensed Practice Nurse (LPN) for a 0.4 FTE temporary Family Practice Nurse position in Westlock, AB . The LPN Family Practice Nurse works collaboratively with the multidisciplinary team, physicians, and community agencies to enhance the delivery of primary care services across all age groups. The LPN Family Practice Nurse works within their scope of practice to provide high-quality, evidence-based care to patients with a variety of medical conditions. The successful applicant must have experience working in a primary care medical clinic. What You Will Do Works to full scope of practice with an emphasis on providing comprehensive care to patients of all ages Specific focus on chronic disease management is a key area of work Provides full health services based on highly developed nursing knowledge and skills Conducts assessments to identify needs and collaborates with patients to set priorities and plans for care Provides short term interventions and patient education Provides assistance with navigation of the health system What You Need To Succeed Current CLPNA registration and CPR Five years or more related nursing experience preferred Experience working in a primary care medical clinic considered an asset Experience working with Microsoft office suite and EMR systems considered an asset Successful completion of Medication Administration and injection courses Advanced skills in health promotion, health screening, assessment and education Strong knowledge and understanding of Indigenous culture and communities considered an asset Ability to practice in a culturally safe manner Security, proof of clean criminal record check Completion of reference checks Copy of current CLPNA registration (submitted annually) Copy of current CPR (submitted annually) To Apply The Aspen Primary Care Network offers a competitive salary and benefits package.
