Senior Support Coordinator Homewood Health Centre Make a difference in a growing, innovative mental health organization and centre of excellence in peoplecentered care. What Youll Be Doing Provide support coordination for evidencebased, complexities within the senior population with serious mental health and concurrent disorders Conduct comprehensive patient assessments Provide support coordination within a recovery framework to include treatment and community supports with both formal and informal networks Ensure clients are supported in community environments, with engagement in community locations at a minimum of 75% of the service period What Were Looking For Bachelors in Psychology, Social Work or other related field Valid registration with the relevant college/governing body in Ontario Knowledge of older adult population needs and concurrent mental health disorders 1 3 years experience in concurrent mental health and knowledge of the complexities within the senior population Valid drivers license, business insurance and personal vehicle Hours of Work Permanent Full Time, 35 hours per week As part of our commitment to the health and safety of our patients, staff and community from COVID19, subject to any accommodation required by applicable human rights legislation, Homewood Health Centre requires that all staff have received all required doses of a COVID19 vaccine approved by Health Canada.
