Case Managers work in partnership with clients to develop the skills and supports they need to live successfully in the community. Clients are supported through visits in their own home, in the community or in the office.The Case Manager works within a Trillium Health Partners community-based case management service for Mississauga residents, aged 18 and over, who have a history of serious and persistent mental illness. They work in partnership with clients to develop the skills and supports needed to live successfully in the community. The successful candidate will meet with clients either in their own home, in the community or in the office to provide recovery-based support.Responsibilities:Manage a full caseload, providing case management support to assigned individualsComplete assessment reports outlining individual psychosocial needs and determining eligibility for the programEngage individuals in the Recovery process through a goal oriented, skills-based approachOngoing monitoring of individuals wellness, assessment of risk and overall progress in ones Recovery ProcessLiaise with existing treatment team including physiciansQualifications:Bachelor's or Community College degree in a health-related disciplineCurrent registration with a recognized college preferredOver 5 years experience working with adult clients diagnosed with serious mental illnessesExperience working with people who are homeless or have frequent contact with shelters an assetExperience working with clients with substance misuse issues is an assetWorking knowledge of community resourcesTraining and working knowledge in the area of RecoveryExcellent communication, organization, presentation and problem-solving skillsTreatment planning and Client intervention experience requiredValid drivers license, relevant insurance and use of own vehicleDemonstrated ability to work independently and as a part of a teamAbility to speak a second language would be a definite assetProven skills in the area of Crisis Intervention and Suicide Risk AssessmentTrillium Health Partners is an equal opportunity employer who values antiracism and is committed to integrating antiracism, diversity, equity and inclusion best practices throughout THP operations, policies and culture.
