Reporting to the Senior Manager, the Nurse Practitioner, NPSTAT position is responsible for supporting initiatives to reduce transfers from long term care to hospital by providing direct care to long term care residents and supporting long term care homes within a designated geographical area. Please note that this position has an attending NP assignment at a designated LTC home (subject to change). What will you do? Manage the delivery of coordinated quality care programs in designated long-term care facilities Work within the full scope of practice and will autonomously diagnose, order and interpret diagnostic tests and prescribe pharmaceuticals within the NP legislated scope of practice to avert transfers to emergency departments and hospital admissions Provide patient assessment, diagnosis, treatment, pain and symptom management to clients. Develop collaborative relationships with long term care home staff, Ontario Health atHome Care Coordinators, physicians, hospital staff and others Manage complex and difficult patient issues which cannot be handled in a routine manner Collaborative consultation with physicians to maintain patient centered care Participate in and provide effective advice and guidance to staff to help manage difficult and unusual cases Support Ontario Health atHome initiatives and implementation into practice, including Behavioural Supports Ontario Support data collection and program review consistent with NPSTAT objectives Design and implement material and strategic for enhance of clinical competency and capacity of long term care staff to address clinical complexity in the long term care environment Mentor and assist in orientating new NPs and support other clinical programs as directed by NP leadership Embody Ontario Health atHome mission, vision and values and apply quadruple aim (enhancing patient experience, enhancing provider/staff experience, improving value and improving populations health) to support continuous quality improvement in daily work Exemplify, embrace and intentionally promote an inclusive work environment where all are meant to feel they belong Continually demonstrate a commitment to create a positive culture of equity, inclusion, diversity and anti-racism What must you have? Masters degree, any relevant field Current RN(EC) registration with the College of Nurses of Ontario (with an NP certificate in Primary Health Care or Adult Care) A minimum of 3 years nursing RN experience Working knowledge of Ontario Health atHomes role in the long term care sector A true passion for making a difference in the lives of our long-term care residents Critical thinking skills Demonstrated leadership, ability to work autonomously but also as a team Collaborative Empathic and patient centered What would give you the edge? Gerontology Nurse Certified Canada (GNC) Emergency department, mental health/dementia-related behaviours and/or geriatric experience Strong clinical experience Hours Of Work Monday to Friday 10am-6pm, subject to change as per the collective agreement (35 hours per week; 70 hours per bi-weekly pay period). Position location and travel Hybrid work model, subject to change. Regular travel within the geographical region of Ontario Health atHome Central East may be required. A valid Ontario drivers license and access to a vehicle are necessary. What do we offer?
