This role plays a critical part in ensuring high quality, coordinated care across inpatient and outpatient settings. The Nurse Clinical Coordinator provides expert consultations and direct care for patients with complex wounds, fistulas, and ostomies, while also supporting patients and families through education and discharge planning. In addition to clinical responsibilities, the role involves active participation in quality improvement initiatives, including program development, monitoring, and outcome evaluation. The Nurse Clinical Coordinator contributes to academic and research projects, supports staff education, and provides leadership in advancing clinical nursing practice. Administrative responsibilities, interdisciplinary collaboration, and cross-functional duties are also integral to this position. The Nurse Clinical Coordinator is expected to collaborate across departments and perform other related duties consistent with the scope of the role. Approximately 60% of the role focuses on developing a comprehensive program for patients within Krembil Neurosciences (Movement Disorders) requiring percutaneous endoscopic gastrostomy tubes, including outreach to community partners locally and nationally.DutiesCoordinate and oversee care for patients with complex wound, ostomy, and skin care needs, ensuring seamless service delivery across care settingsPerform expert clinical assessments, provide consultations, and develop individualized care plans in collaboration with interdisciplinary teamsDeliver patient and family education related to ostomy and wound care, and develop tailored educational materialsLead and support quality improvement initiatives, evaluate clinical outcomes, and foster the integration of evidence-based practicesContribute to research and academic initiatives, maintain accurate clinical documentation, and support workflow improvementsAct as a clinical resource within the organization and build capacity within our teamsQualificationsMasters degree in Nursing preferredRegistered and in good standing with the College of Nurses of OntarioCompletion of Basic Cardiac Life Support (BCLS) program, Level II Rescuer certificationAdvanced education program such as NSWOCC, MClSc-WH, MScCH: WPC, WOCN, or IIWCCCNA Certification in Wound, Ostomy and ContinenceMembership in RNAO or other relevant organizations preferredFive (5) years of relevant experience preferably in an acute care settingTwo (2) or more years in the specialty of wound, ostomy and continence careAbility to perform duties professionally, compassionately, and with high quality while meeting deadlines in accordance with UHN standardsInitiative, good judgment, and ability to multi-taskCompliance with confidentiality requirementsEffective organizational, interpersonal, and communication skillsKnowledge of applicable policies and regulationsExcellent teaching skills for one-on-one and/or group settingsAbility to work collaboratively within a multidisciplinary team and/or independentlyAbility to use Electronic Patient Record systems effectivelyResearch experience is an assetAdditional InformationWhy join UHN?
