Job Description

RESPONSIBILITIES Develops, coordinates and implements in-home nursing services under the direction of the HCC Nurse Manager, and in conjunction with KO community Nurse in Charge and community Health Director. Works with a team of Personal Support Workers (PSW''s), unregulated healthcare providers, and other healthcare professionals in the program. Maintains a relationship with the medical professionals in the community, the Chief and Council, and other stakeholders. Maintain current skills through attendance at workshops, in-service and staff meetings. Assists the HCC Nurse Manager with preparing reports for the KO Director of Nursing (DON) and annual year end reports for Health Canada. Performs other duties requested by HCC Nurse Manager, KO Health Director, or DON. Charting using the Tellus. KNOWLEDGE, SKILLS, AND ABILITIES A Registered Nurse (RN)/ Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario. Minimum 2 years'' experience delivering nursing care in a home or community setting. Advanced Foot Care certified; Mental Health nursing an asset. Knowledge on culture and tradition of remote First Nation communities. Excellent verbal and written communication skills. Ability to speak Ojibway, Oji-Cree, or Cree an asset. Ability to prepare and submit reports. Basic computer skills and willing to learn a new Tellus system. Awareness of working with unregulated Health Workers; Transfer/Delegation of Skills, processes and requirements. Other duties as required Schedule: Rotational basis 2-4 weeks in communities.
