PURPOSE The Community Health Ambassador plays a key role in community health initiatives, advocacy, client engagement, population health while working within WellFort''s Open Focused Care service delivery.The Community Health Ambassador coordinates and implements the Locally Driven Population Health program and Culturally Responsive Prevention Model, while working collaboratively with community groups, networking agencies and other stakeholders using a community development model.The Community Health Ambassador works with inter-disciplinary teams functioning both independently and with the community team in health promotion, education, community development, advocacy and outreach activities. This position requires flexibility to support accessible program service delivery across various sites and programs based on care delivery needs. RESPONSIBILITIES Working closely with the primary care team, interprofessional team, and the community services team at WellFort, the Community Health Ambassador will play a key role of reaching out to community members and provide relevant and accurate information about the importance of preventative primary care, diabetes care, preventative cancer screening, and vaccination. Provide culturally appropriate education and information about cancer screening; diabetes care; and vaccination. Help facilitate primary care, cancer screening and diabetes appointment bookings through an internal and external referral process. Conduct educational presentations within the community/community organizations Track and collect daily outreach and education activities and ensure data is up-to-date. Work closely with the Support and Care Navigators, and Health Promoters to inform the creation of educational resources and outreach strategies Promote and recruit community members for targeted focus groups and interview sessions The Community Health Ambassador will also direct community members to the right service and programs for support including food security support, access to financial assistance, access to mental health support. Educate community members and clients about the COVID vaccine and its importance. Conduct grass root community outreach activities Liaise with relevant committees/networks to assist engaged members of the community to access practical and emotional support services provided by WellFort or by other community partners Participate in the collection of trends, outreach activity statistics and project evaluation activities Participate and support in educational sessions, town halls and other health promotion activities related to vaccination, preventative cancer screening and diabetes care. Provide information and referrals to internal and external resources as required. Participate in events that educate the community on issues related to preventative care and Diabetes Provide community outreach using personal vehicle or local transit. Upon the organizational and provincial needs, you may be required to assist with duties in other programs, mobile clinics, and outreach or to complete other responsibilities as assigned. Engage in proactive outreach to promote WellFort health promotion initiatives, programs and services Participates in community projects or WellFort activities to provide education, information or to develop collaborative strategies. Work with teams to support the development of content for WellFort''s Website, and Social Media accounts, related to Health Promotion, Advocacy and Special Campaigns. Complete administrative duties as assigned Other duties and responsibilities may be assigned from time to time. QUALIFICATIONS, EXPERIENCE & SKILLS Undergraduate degree or college diploma in a relevant field, or a combination of lived/living and work experience relevant to the role. Experience with community-based outreach in an employment or volunteer capacity. Fluency in South Asian, African, or Latino languages is a significant asset. Demonstrated knowledge, skills, and abilities in social service or community work, including asset-based community development approach to outreach activities. Lived experience of working with the Black, African and Caribbean (BAC), South Asian, Middle Eastern, and Latino communities of different age, gender and socio-economic status Demonstrated knowledge of myths, misinformation, and stigma, as it relates to preventative care, cancer screening and diabetes within the BAC and South Asian Communities Strong knowledge of community resources, health promotion and the impact of social determinants of health and public health policies on the racialized and marginalized communities Proven time management and organizational skills. Experience in group work, program development, implementation, monitoring, and evaluation. Excellent written and verbal communication skills, along with group facilitation abilities. Experience in developing and facilitating group presentations. Concrete knowledge of and strong ties to the communities and neigbourhoods within Bramalea and Malton (L6T, L6S, L6R, L6P, L4T) Valid Ontario Driver''s license and access to a vehicle. Ability to work independently and collaborate effectively in an interdisciplinary team environment.
