Job Title: Palliative Pain & Symptom Management Consultant Job Type: Full Time, 37.5 hours/week Wage Rate: $38.57-$48.08 per hour Position Summary: Reporting to the Director of Hospice, the Palliative Pain & Symptom Management Consultant delivers case-based consultation, education, training and mentorship to staff providing palliative care in a variety of care settings. Facilitates palliative care courses, seminars and other education opportunities using principles of adult education, and builds community capacity within the areas of Brantford, Brant, Norfolk, Haldimand, Six Nations of the Grand River and Mississaugas of the Credit First Nation ensuring the highest quality of care is provided to all, and in keeping with the Mission, Vision and Values of Stedman Community Hospice/St. Joseph''s Health System. How You Will Make a Difference: Palliative Pain and Symptom Management: In collaboration with the Palliative Clinical Coaches, assist service providers in the application of the Model to Guide Hospice Palliative Care, assessment tools and best practice guidelines Offer consultation to service providers in person, by telephone, by videoconference or through e-mail regarding care, e.g. assessment and management of pain and other symptoms Provide case-based consultation, education and mentoring for service providers in a variety of settings including; Long Term Care Homes, Retirement Homes, Hospitals, Community Nursing and Support Agencies, Hospices, Day Programs, Supportive Housing and other settings Help build capacity amongst front line service providers in the delivery of palliative care Link providers with specialized hospice palliative care resources Collaborate with local community-based Palliative Care Teams providing linkages to resources, other organizations and supports. Community and Facility Palliative Care Interdisciplinary Education: In collaboration with the Palliative Clinical Coaches (when possible): Provide/coordinate education to front line health care providers Support learners through ongoing coaching and mentoring Incorporate principles of effective adult learning into all education sessions Community Enabler/Capacity Building: Collaborate with community care providers to identify gaps in palliative approaches to care and recommend potential solutions Receive and collate information from health care providers at the community level that relates to gaps, issues and potential solutions Communicate these issues and potential solutions to the appropriate advisory tables/committees Support the implementation of strategic initiatives that provide solutions to the issues, which will include education of health care providers Participate in evaluating the efficacy of various initiatives Network with Palliative Care Consultants and Clinical Coaches across the region to support one another and to ensure consistency (but not uniformity) across programs. Additional Responsibilities: Contribute to data collection and quality improvement programs Contribute to the development of policies and procedures Embody St. Joseph''s Health System mission, vision and values and apply quadruple aim (enhancing patient experience, enhancing provider/staff experience, improving value and improving populations health) to support continuous quality improvement in daily work Exemplify, embrace and promote an inclusive work environment; and Demonstrate a commitment to create a positive culture of equity, inclusion, diversity, anti-racism and Indigenous Reconciliation Qualifications & Skills: Educational Qualifications: BScN with a Current Certificate of Registration with the College of Nurses of Ontario as a Registered Nurse, (RN or RN/EC) with palliative expertise and training, or equivalent CNA Certification in Hospice Palliative Care Nursing desired Membership with RNAO Professional Experience: Minimum three to five years'' experience in palliative care in a variety of care settings, with demonstrated palliative pain and symptom management experience Strong organizational skills with demonstrated initiative and leadership in managing and prioritizing administrative responsibilities. Additional Knowledge and Skills: Highly developed interpersonal, communication, teamwork, organization, coordination and time management skills Demonstrated proficiency working with IT platforms including (but not limited to): MS Office Suites, MS Teams, Canva, InfoAnywhere, Point Click Care, Telus Solutions Demonstrated commitment to interdisciplinary care Demonstrated skill in problem solving and critical thinking Demonstrated commitment to continuous learning and professional development Demonstrated knowledge and expertise in facilitating adult education for a variety of health disciplines Ability and willingness to travel within Brantford, Brant Norfolk, Haldimand and the surrounding areas, to a variety of care settings Ability and willingness to work flexible hours including some evenings and weekends to meet the needs of the program Experience collaborating with community partners in facilitating palliative care is an asset Experience mentoring learners in a variety of care settings is an asset Health & Safety Requirements: Follow safety-related policies, procedures, and protocols, including the use of personal protective equipment. At all times follow the requirements of Stedman Community Hospice policies and procedures, the OHSA and applicable Hospice standards/ regulations in carrying out the duties and functions of a Stedman Community Hospice Employee, including reporting workplace incidents and /or hazardous situations to the appropriate supervisor in a timely basis, and participating in the resolution of these situations. Attend training required by Stedman Community Hospice and apply the instruction and learning to protect self, coworkers and patients Use or wear protective equipment, devices and methods as outlined by Stedman Community Hospice and work in a manner that promotes the safety to self, coworkers and patients.
