The Nurse Case Manager is the clinical backbone of a specialty patient programcombining patient education, benefit navigation (as applicable), scheduling/coordination, and close communication with prescribers, pharmacies, and other stakeholders. Youll help patients start therapy successfully, maintain continuity of care, and feel supported throughout their treatment journey.What Youll Do (Key Responsibilities)Enroll & welcome patients: Complete onboarding and welcome calls following defined scripts; ensure accurate capture of clinical/admin details and consent.Coordinate care: Schedule diagnostics/appointments; follow up on expired or missing test results; verify next appointments; track missing postvisit reports (where applicable).Educate & support: Provide therapyrelated education to patients and HCPs; offer and/or deliver product training (as program permits).Reimbursement support: Where programspecific, liaise with payors to determine coverage options; advocate and follow up until a decision is received.Stakeholder liaison: Maintain transparent communication with physicians, nurses, PSP teams, pharmacy partners, and manufacturer field teams to resolve barriers.Documentation & data integrity: Maintain accurate patient records; ensure data integrity to meet program reporting requirements.Compliance & safety: Identify and report AEs to appropriate channels; complete product complaint reports (as applicable); adhere to privacy regulations and Quality Management System practices.Operational excellence: Complete time/expense reports (if required), participate in training and continuous improvement, and support program KPIs.
