The Health Promoter for the Mobile Health Clinic (MHC) & Health Access Taylor Massey (HATM) coordinates and implements health education/promotion/social prescribing programs, and facilitates the integration of a health promotion focus in HATM activities as a member of a multidisciplinary team. In addition, the Health Promoter will work in the Taylor Massey and Oakridge Neighbourhoods with a primary focus on attaching community members to Primary Care either at HATM or on the MHC. The Health Promoter also works collaboratively with community groups and agencies using a community development model. This position is further expected to be available at hours that are suitable for community members and volunteers. This may mean early morning and evening hours as well as weekend time. Responsibilities Coordination of Health Promotion Programming Supports service coordination, outreach and engagement between SRCHC, MHC, HATM and SRCHC programs and services to increase access and attachment to team based primary care for priority populations Collaborates and consults with multidisciplinary staff, partnering organizations and broader community to support program planning and development Networks and works collaboratively with community groups to strengthen and expand health promotion programming, developing and maintaining formal and informal networks Coordinates, communicates and promotes the activities or initiatives associated with HATM, the MHC and all partner agencies and the community related to expanding access to primary care Health Education Works both at HATM and on the Mobile Health Clinic with diverse populations including individuals who are homeless/precariously housed and/or have mental health and/or substance use issues Identifies client needs and priorities and works with team members to make appropriate internal/external referrals including social prescribing as needed Uses techniques and approaches that support crisis intervention and are traumainformed, strengthbased, culturally competent, and appropriate for client needs Supports the operation of the Mobile Health Clinic on a regular basis, including driving the MHC to community locations, helping with setup and managing the mobile clinic environment etc. Acts as a frontline support to assist clients with complex mental health and social issues by referring them to appropriate resources Collaborates with other health providers to provide a model of holistic, interdisciplinary team Writes reports, maintains files, and collects required data for program monitoring and evaluation Identifies, in partnership with community members, health related education needs and plans, develops, implements and evaluates health education programming - using various formats such as community events, group programming, use of media and presentations to large groups Disseminates accessible health education materials, developed with community members Collaborates with all teams at HATM to identify and provide health education to target groups Ensures the Mobile Health Clinic is organized, clean and presentable to clients and the public Conducts mandatory circle checks on vehicle(s), noting minor and major vehicle defects, topping up vehicle fluids (e.g. gas and windshield wiper fluid) in a timely manner. Reports to supervisor any concerns regarding the condition of the vehicle(s) which may make them unsafe to drive Advocacy Advocates on behalf of priority population groups and individual community members, for accessible relevant services Collaborates and builds partnerships around issues of concern to effect change Participates in systemic advocacy initiatives directed at providing supports for clients with complex needs impacted Social Determinants of Health All other duties as required For full position details and application instructions, please refer to our website. #J-18808-Ljbffr
