Client Care Coordinator In-Home Full-Time | Hybrid | Travel Required (York Region & South Simcoe) CHATS is seeking a passionate, organized, and people-focused Client Care Coordinator In-Home to support the delivery of high-quality in-home services for seniors through comprehensive assessments, documentation, coordination, and collaboration across interdisciplinary teams. The Role Reporting to the Client Services Manager In-Home Team, HAMP (Home Adaptation & Maintenance Program) & TRRS (Tenancy Risk Reduction Services), this role plays a key operational and coordination function by conducting client needs and risk assessments, supporting ongoing case oversight, and collaborating closely with Client Care Supervisors and the broader In-Home team. You will primarily support in-home services, with occasional involvement in community programs when assessment or case management support is required. This position requires regular in-person client visits, strong documentation skills, and the ability to manage multiple priorities in a fast-paced, client-centered environment. This is a full-time position (35 hours per week), Monday to Friday, 8:30 a.m. 4:30 p.m., with occasional work outside regular business hours. This hybrid role involves working remotely, in the office, and in the community. Travel within York Region and South Simcoe is required, with a base at CHATS Head Office in Aurora. Key Responsibilities: Conduct timely, comprehensive client needs and risk assessments, with a strong focus on client safety and well-being. Collaborate with Client Care Supervisors to identify clients or situations requiring priority home visits. Provide oversight and support for priority waitlists and assessment processes. Support the delivery of in-home services through excellent customer service, team collaboration, quality improvement, and risk management. Provide operational and administrative support to the In-Home team, contributing to individual and team performance. Maintain accurate documentation, tracking, and monitoring of services using GoldCare and the In-Home Management Dashboard to support service delivery, reporting, and operational oversight. Collaborate effectively with Client Care Supervisors, Client Service Coordinators, and PSW/HSW colleagues to support integrated service delivery. Participate in community-based work as required to support assessments or case management needs. What You Bring to the Role Social Service Worker-Gerontology or related education, combined with three years'' experience with seniors and/or provision of client services in the community Strong assessment, interpersonal communication, and problem-solving skills. Excellent verbal and written communication and documentation abilities. Demonstrated skills in risk, crisis, and conflict management. Current CPR and First Aid certification. Proficiency in MS Word, Excel, Outlook, and database applications (GoldCare experience an asset). Valid Ontario Driver''s License and access to a reliable vehicle. Ability to work outside regular business hours when required. Fluency in English and French and/or additional languages is considered an asset.
