Program Overview Community Care, Seniors Active Living Centres & Outreach This community-based seniors-focused FVME program is designed to promote wellness through matching volunteers with clients for in-home visits or as escorts for medical appointments. The FVME Coordinator is responsible to work closely with program participants in developing, organizing, facilitating, and evaluating programs for seniors; to outreach to seniors in the community; to develop new programs that meet emerging needs; to provide program administration; and to assist the Manager in developing policies and procedures to improve the program delivery system. What You Will Do Develop, implement and evaluate the Friendly Visiting & Medical Escorts program which promotes wellness through matching volunteers with clients for in-home visits or as escorts for appointments. Identify emerging visiting needs and develop new programs, in collaboration with the Manager, and aligned with the goals/objectives of the Friendly Visiting & Medical Escorts Program. Through the Medical Escort support service help isolated, frail, and otherwise vulnerable seniors go to medical appointments by accompanying them from their home to the doctor''s office and back as requested. Complete and maintain all records and forms regarding membership and program/activity enrollment, using a client database. Develop and review the FVME program manual/operational procedures to ensure that service standards are met. In consultation with the Manager, respond to community needs, ensure existing and new program development meet these needs. Work with the Volunteer Services in recruiting, training, and reference checks of Volunteers before matching with clients. As a team member of community programs utilizing volunteer based activities, manages and oversees day to day programming. Maintain and develop contacts with other seniors'' groups and community agency and attend external meetings as recommended by the Manager and/or Unit Director. Work effectively with members of the unit and other team staff. Conduct initial home visit and assess the needs of each client, using the required assessment tools as per the Unit''s assessment guidelines. Support day-to-day programs and Food Security initiatives as needed. Responsible for representing the unit on internal staff committees as directed by the Unit Director.Ensure the anti-racism objectives and community development principles of the organization are achieved within the program. Participate in regular supervisory sessions with the Manager. Responsible for upgrading training and education as directed by WoodGreen Community Centre to fulfill the responsibilities of the job. Familiar with all of WoodGreen''s emergency procedures and staff telephone numbers. Perform other related duties as required from time to time. What You Bring to the Team Bachelor''s degree in social work or related major; Volunteer Management Certificate an asset 2 3 years related experience managing volunteers. What Will Set You Apart Good understanding and knowledge of seniors'' issues. Good understanding of social and wellness related to aging. Knowledge and demonstrated skills in program development and management. Able to work independently with minimal supervision. Good multi-tasking and strong research skills. Knowledge of community resources for older adults; interest and experience in event coordination. Second language is an asset.
