With the anticipation of opening a new building in March 2026, we are searching for a Registered Nurse to join the Shannex Transitional Health Services Limited team providing transitional care services at West Bedford Transitional Health in West Bedford, Nova Scotia. Through an innovative partnership with Nova Scotia Health (NSH), Shannex proudly operates the provinces first transitional health facility in West Bedford. This facility provides specialized care for NSH patients in the central zone who no longer require hospital care but need additional support and services before safely returning home or to their community. Reporting to the Manager of Transitional Services, the RN is integral in supporting the goal of returning to home/community by being engaged in interprofessional collaboration, offering evidencebased clinical expertise to inform care plans that fit with patient/family goals, using effective communication strategies, shared decisionmaking, and mutual support. Perform initial and regular health assessments to maintain patient quality of life. Plan, implement, and evaluate nursing care of all patients. Manage patient medication in accordance with legislative requirements. Follow the process in place for safekeeping of medications. Administer and record prescribed medications in accordance with facility policy. Ensure all completed care tasks are documented appropriately using identified technology and tools. Ensure Continuing Care Assistants/PSWs are completing care in a timely manner. Ensure Continuing Care Assistants/ document all completed care, incidents, and errors accurately. Apply dressings and treatments according to physicians orders and facility policy. Develop, implement and evaluate care plans. Observe, record and report symptoms, reactions, treatments, and changes in the patients condition. Document according to facility policy e.G. Daily Patient Care Record, Medication Administration Record (MAR), patient progress notes, incident reports and other facility specific documentation as required. Support continuous quality improvement, risk management and quality assurance activities. Ensure organizational policies are adhered to by team. Ensure patient safety by ensuring team members follow and apply all standard operational procedures, safe work practices and training provided. Additional duties as required About You Bachelor of Science in Nursing and currently registered with the Provincial Association of Registered Nurses. Proof of valid CPR/First Aid certification required. Ability to provide a clear criminal record and vulnerable sector check upon hire. Demonstrated supervisory skills and ability to evaluate employee job performance. Specialized courses such as Palliative Care, Venipuncture and Dementia: Understanding the Journey considered an asset. Previous experience supporting admission and discharge of vulnerable populations with complex social situations; implementing careplansand identifying resources that will give the patient every opportunity to be successful in returning to home/community is considered an asset.
