Job Title Registered Practical Nurse (subsequent) Status Regular Part Time Department LG LTC 6W RESIDENTIAL Program Long Term Care Schedule Days, Evenings, Nights, Weekends; 8 Hour Shifts (Subject to Change) Location Lakeridge Health Long Term Care AP Wage Schedule $36.140 - $38.160 per hour Posting Reference 2500002655 Position Summary Reporting to Nursing Leadership, the Registered Practical Nurse will provide excellent nursing care to residents at the Lakeridge Gardens 320 bed long term care home being built on the AjaxPickering hospital site. This new home is part of the governments plan to improve longterm care delivery and capacity in Ontario and in the Durham Region. This stateoftheart longterm care home sets a new bar for seniors care, with dining and outdoor space on every floor, a large courtyard, onsite diabetes care and other important innovative features. Just as Lakeridge Health is a beacon in the Durham community, the new longterm care home will be integral to managing and delivering on the needs and expectations of seniors in the Durham community and Ontario. Duties and Responsibilities Assessing, planning, implementing and evaluating to provide high quality nursing care to residents; Assisting in the development and maintenance of an appropriate plan of care for each resident Ensuring resident care plans are initiated in collaboration with other members of the multidisciplinary team; Communicating with relatives regarding the care and condition of respective residents as required; Participating in discharge planning, consultation, and/or transfer of residents; Providing direct resident care and participate in resident teaching as required; Encouraging active participation by residents in all therapy programs and recreational activities; Providing first aid to staff when necessary and completing Employee Injury Reports; Procuring and administering medications including injections and narcotics; Participating in the orientation, onthejob training and inservicing of nursing staff as required. Qualifications Current Certificate of Registration in good standing with the College of Nurses of Ontario as a Registered Practical Nurse; Related experience in a geriatric or Long Term Care environment is preferred; Demonstrated commitment to maintain Reflective Practice Standards as outlined by the CNO; Demonstrated knowledge of and compliance with safe work practices; Must possess strong written and oral communication, critical thinking, care coordination, time management and customer focus skills; Demonstrated empathy, concern and interest in providing quality care and service for residents; Physical ability to perform the essential duties of the job; Must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months; Demonstrated commitment to our core values of Inclusion, Compassion, Innovation, Teamwork and Joy; Demonstrated understanding of and commitment to Lakeridge Healths comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular inservices in this area.
