Required Availability: At least 3-5 days per week including weekends 8:00 a.m. 4:00 p.m. or 3:00 p.m. 11:00 p.m. Work Location: Oakville/Milton Work Setting: Community Care Responsibilities Arrange and/or provide client/patient care by utilizing observation, assessment, nursing diagnosis, counseling, and health teaching; report unusual circumstances to the branch office or immediate supervisor. Develop and review nursing care plans that are client/patient-centered, goal-oriented, and individualized to each client/patient and their family. Provide care according to the established care plan, ensuring emotional, spiritual, physical comfort and safety of clients/patients within the scope of practice of a Registered Nurse. Perform routine nursing duties per company policies, Standards of Nursing Practice, and specialized procedures with demonstrated competency. Observe and document client conditions according to Bayshore Home Health standards; receive and transcribe physician orders. Provide direction, support, and delegation to Licensed/Registered Practical Nurses and paraprofessional staff (PSWs, HCAs, CHWs, etc.) as appropriate. Understand and implement infection prevention and control practices. Engage in ongoing internal/external continuing education to maintain full scope of practice and competence. Maintain knowledge of best practices, provincial regulatory requirements, and RN scope of practice. Identify when client care needs exceed nursing competency and escalate accordingly. Adhere to Bayshore Policies and Procedures and participate in quality improvement activities within the Quality Management System. Maintain privacy and confidentiality of client and corporate information; discuss with appropriate Bayshore personnel. Maintain knowledge of ICH, GCP, and Health Canada GVP guidelines relevant to the role; apply knowledge accordingly with regular updates. Report and document Adverse Events per Pharmacovigilance requirements. Safety Responsibilities Comply with Bayshore Health and Safety policies and procedures, including Infection Prevention & Control. Work safely and report injuries, near misses, or hazards to the supervisor. Maintain a safe working environment and participate in emergency drills as required. Review and adhere to workplace harassment and/or violence policies. Current CPR certification required. Other Requirements Clear criminal background check (with vulnerable sector screening) and negative TB test.
