What We Offer: Nurse Next Door People Promise: you love coming to work 24/7 support to assist Flexible scheduling with guaranteed weekly hours and opportunity for growth One-on-one in-home care with private clients Responsibilities And Duties Consult Conversion Conduct Caring Consults, assess clients with respect to physical, emotional, clinical and environmental needs and advise on client care; achieve conversion goal of 90% Develop relationships with community leaders and professional organizations to establish long-term relationships for referrals Develop, promote and ensure high quality, innovative customer service Act as advisor in Caring Consult to ensure client and contacts have all information pertaining to care and services provided Client Care Delivery and Experience Attend all First Visits with caregivers to ensure client needs and care plans are executed as per the service agreement; set and model expectations for caregiver Conduct Supervisory Visits to ensure caregiver fit and quality of care; deliver a premium client experience impacting Net Promoter Scores of 9 or higher Develop appropriate and individualized care plans and update as required Educate clients on self-care and evaluate client response to care provided Monitor risk issues and analyze trends, adapting care as needed Coordinate care, implement clinical solutions, and evaluate client outcomes based on client goals Responsible for client care as needed People Leadership Lead the caregiving team by setting clear goals, assigning tasks, coaching in the moment, and providing continuing education as needed Deliver on the Brand Promise by leading the client experience and ensuring caregivers go above and beyond to bring peace of mind to clients and families Conduct First and Supervisory visits to evaluate caregiver performance and job satisfaction, impacting the employee experience with Net Promoter Score of 9 or higher Verify caregiver skills on site as needed pertaining to the care plan Scheduling Collaboration Coordinate scheduling with the General Manager to determine matching, communicate changes in client requirements, or caregiver assignments Ensure client coverage is secured, especially for urgent care, caregiver cancellations and limited availability; assist with scheduling as needed Maintain data integrity and completion of client files Administration Maintain excellent documentation practices Review and update client records quarterly, or as needed, to ensure 100% accuracy and completeness Ensure progress notes are reviewed and provide necessary coaching to caregivers Answer phone calls as needed and document all intakes appropriately, including entering into AlayaCare and ensuring proper follow up Requirements Experienced Registered Practical Nurse, licensed to practice in the Province of Ontario BLS Certification Valid drivers license (will have access to company vehicle) Strong computer skills, ideally proficient in G-Suite: Gmail, Drive, Docs, Calendar Exceptional written and verbal communication skills in the English language Bilingual Intentional organization, planning, time management and multitasking skills Demonstrated experience setting and achieving goals Customer service and leadership background considered an asset Up to date Criminal Record Check with Vulnerable Sector Up to date Negative TB test Flexible availability, with evenings, weekends and some holidays included Rate of pay negotiable based on experience Nurse Next Door is an Equal Employment Opportunity employer.
