BACKGROUND The Vaughan Community Health Centre (VCHC) is a not-for-profit, community-governed organization providing primary health care, social services, and illness prevention programs primarily to the residents of the Western and Northern York Region. We serve people experiencing barriers to accessing health care services due to culture, language, age, chronic illness, or the like. We are committed to providing services with and for local Indigenous, Black, racialized and other disadvantaged communities. POSITION SUMMARY As a member of the interdisciplinary team, the Chiropodist works to provide foot care services within the scope of their license. The care modality emphasizes health promotion and client education. The Chiropodist serves clients from the Primary Care Team and the People Accessing Care Teams (PACT) program at the Vaughan site. KEY ACCOUNTABILITIES AND RESPONSIBILITIES Perform clinical duties by assessing clients for routine foot health and illness care, treating, and preventing disease, dysfunction, or disorders of the foot by therapeutic, surgical, palliative or orthotic means, as well as prescribing medications within the scope of the license. The performance of surgical procedures includes the injection of prescribed medications. Ensure appropriate, comprehensive treatment is delivered by maintaining complete and accurate documentation in the electronic medical records system; participating in chart reviews and discussions of problem cases; and answering client enquiries and providing the appropriate treatment. Work with Primary Care Team, PACT Team and Senior Chiropodist to develop, implement, and evaluate chiropody services. (i.e. provide services to clients experiencing barriers; offer number of follow-up visits based on PACT intake criteria; follow timely discharge process and procedure for clients, etc.) Make interdisciplinary referrals within VCHC team as appropriate to provide quality care to clients. Ensure that up-to-date infection prevention and control (IPAC) practices are followed, and chiropody resources and equipment are maintained. Chiropody services are provided during regular scheduled hours and may include home visits as required to VCHC primary care clients only. Implement workflow processes in alignment with VCHC chiropodists. (i.e., health outcome data, alignment of documentation practices, etc.) Identify areas where the development of protocols and procedures are needed to improve client services or to promote more effective staff functioning. Participate in Centres quality improvement initiatives for e.g., quality improvement plan, peer chart reviews as identified based on best practice guidelines and Vaughan CHC priorities. Maintain and develop professional competence through appropriate continuing education (i.e., peer interaction, literature review, conferences, courses, etc.) QUALIFICATION REQUIREMENTS Graduate of an accredited school of Chiropody or Podiatry Active registration and in good standing with the College of Chiropodists of Ontario; and proof of professional liability insurance. Thorough knowledge of and proficiency in the underlying theory and practical application of chiropody One to three years of experience in a community-based and/or hospital setting. Demonstrated ability to work effectively in a multidisciplinary team environment. Experience working in an Electronic Record System (EMR) The ability to communicate in a second language relevant to clients living in Western and Northern York Region is an asset.
