Practice Information Practice Name: Forward Care Medical Clinic Main St Practice Setting: Family Practice Other Providers: MOAs, RN, Specialists Community: PCN 5: Midtown EMR: MEDAccess Clinic Overview Forward Care Medical Clinic Main St is located in a prime area with many excellent restaurants and coffee shops nearby. A bus stop is conveniently located right in front, and the clinic offers street parking for patients. Just a fiveminute walk from LifeLabs and have a large pharmacy directly across the street. They have 8 exam rooms and 1 staff room for charting. Within the clinic, they have a group of inhouse specialists, including two Endocrinologists, an Ophthalmologist, GP-Dermatologist, Nephrologist, Geriatrician, and womens health physician. They also offer services such as Holter monitoring. The team includes RN support and MOAs: one serves as a runner, assisting with patient intake, vitals, and preparation for physicians; one specializes in physician billing to manage any issues that may arise along with front desk MOAs. The clinic provides fullspectrum care, seeing patients of all ages, including children, adults, and elderly patients, as well as supporting postnatal care. Hours Of Work Up to the physicians preference. Open to both fulltime and parttime arrangements and are flexible with hours. (office hours: 8:45 AM 5:00 PM, close on Weekends and Stat Holidays). Typically, twothree days of inclinic patient care and one day of virtual care per week, adjustable based on the physicians preference. On-Call Hours Physicians participate in a rotating pager oncall schedule, shared monthly among providers. #J-18808-Ljbffr
