Patient Panel Manager-Part-time Temporary (12 months) 0.5 FTE Kalyna Country PCN is seeking one temporary part time, Patient Panel Manager (PPM) (0.5 FTE) 12 month term with potential of becoming permanent. The successful candidate will be working with a multi-disciplinary team at the KCPCN St. Albert site to positively impact the health of our clinic patients. Our clinic can be chaotic at times and may require critical outside the box thinking to make quick positive changes. Management is remote and not always on site with team members therefore, independent working skills and strong leadership skills are a must. Key Responsibilities: Works with all members of the clinical care team in a coordinated and collaborative effort to engage, encourage and support patient health enabling the physician to spend more time providing care to patients and less time performing non-physician activities. Utilizes health information technology to screen for preventable diseases including, but not limited to: cardiac health (BP, lipids, wt., BMI) asthma and COPD management communicable diseases (immunizations) lifestyle related (tobacco and alcohol use, obesity, exercise) Collaborates with Physician/NP on recommend preventative care and follow up actions such as referral to other interdisciplinary team members. Education/Experience: Medical Terminology certificate or related experience. Medical Office Assistant or related experience. Experience in a healthcare setting. Experience using an Electronic Medical Record (EMR)(ACCURO). Quality improvement experience Excellent computer and IT skills. Demonstrated and effective communication skills Ability to multitask Willingness to work in a dynamic environment that requires flexibility and balancing of competing priorities Valid drivers license This is a Temporary 12 month term position.
