Job Summary We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes and assisted living facilities. This role requires a strong foundation in nursing practices, excellent communication skills, and the ability to work effectively with diverse patient populations, especially, seniors, and individuals with special needs such as dementia or hospice care. The LPN will be responsible for implementing care plans, administering medications, and supporting overall patient well-being. Responsibilities Deliver comprehensive patient care in accordance with physician orders and facility protocols. Administer medications and treatments accurately and safely. Monitor patient vital signs and observe for changes in condition. Assist with activities of daily living (ADLs) for patients of all ages, including senior populations. Document patient information accurately using electronic health record systems Collaborate with interdisciplinary teams to develop and implement individualized care plans. Perform phlebotomy and laboratory specimen collection when required. Provide education to patients and families regarding health management and medication usage. Support pain management strategies and case management efforts for complex cases. Ensure compliance with healthcare regulations, safety standards, and infection control protocols. Requirements Valid Licensed Practical Nurse (LPN) license in BC Prior experience in nursing home, hospital, acute care, dementia care, home care, hospice care, or assisted living preferred. Knowledge of medical terminology, medication administration procedures, and patient care techniques. Experience with electronic documentation is highly desirable. Strong communication skills and the ability to work collaboratively within a healthcare team. Ability to handle sensitive situations with professionalism and compassion. Familiarity with pain management strategies and case management processes is advantageous. Ability to adapt to various clinical environments and patient needs.
